3 SNE-PTN Our role is to facilitate clinical practice improvement activities that promote the adoption and integration of best practices and continuous quality improvement. We strive to transform your practice into a more efficient, successful and fulfilling experience for you and your patients. Active participation with SNEPTN fulfills the Improvement Activities requirement of MIPS*. PTNs have been asked to support practices in the changes to Medicare Reimbursement Practices are required to submit data to MIPS Directly Practices are also required to report activities to a PTN if they intend to get credit for the Improvement Activities component of MIPS MIPS: The Merit-Based Incentive Payment System. 4 Transformation Plan: Practices that have submitted their PATs will receive an (more information below) allowing them to access an online tool that will allow SNEPTN to determine which initial clinical and transformational goals will work for practices Individualized links, containing an online submission form, will be sent to your address on Monday, March 13. You will need to complete the plan contained in this link by Friday, April 7, If you have not been receiving communications from SNEPTN, then to ensure delivery please check your SPAM folder or to confirm your address on file. If you us, please provide your practice name and zip code, and if possible your Vision Source ID number. 4

5 Transformation Plan: What is the transformation plan and why are we doing it? The Transformation Plan is a tool that sets the foundation for our co-operative goals. (CMS-SNEPTN-Vision Source Practice) The Transformation Plan is a pivotal tool utilized in combination with data collection to establish baselines and allow you to focus on specific, measurable, achievable, relevant, and time-bound goals that will benefit your practice. 5

6 Transformation Plan Walkthrough Completing the Questionnaire: We are employing a secure online submission form to ask practices a series of questions that will allow us to better understand: How ready your practice is for transformation What goals will work best for your practice Where we (SNEPTN) should focus our resources to better serve you If you are unable to complete the survey in one sitting, you can save your progress and return to complete the survey at a later time. Remember to save your validation code. 6

This will allow SNEPTN to accurately match your survey results to your enrollment and submitted Practice Assessment Tool. *** The individual filling out the form and their accompanying address will be utilized as the practice contact and all further correspondence will be sent to that address *** 7

8 Transformation Plan Walkthrough Clinical Measure: As referenced earlier, we are asking you to track and submit a clinical measure to us to facilitate quality improvement. This section of the survey allows SNEPTN to better understand your practice s ability to produce information that will allow you to assess and improve upon the clinical measure. By identifying the groups strengths and weaknesses we can target our efforts to facilitate your improvement process. Even if your practice is NOT MIPS eligible in 2017, we strongly encourage you to participate in our program so you can obtain the skills necessary to succeed as future payment reform initiatives are enacted. 8

9 Transformation Plan Walkthrough Clinical Measure: This section will allow SNEPTN to identify your practice s ability to collect data on your diabetic patients. Many practices will already have experience with this data since retinal or dilated eye exams for diabetics has been a longstanding PQRS measure. Your responses to these questions will assist us in our development of further resources to support you. Submitting this data has three specific benefits for your practice: 1. It establishes the size of your diabetic patient population 2. It allows your practice to track and submit data to SNE-PTN for quality improvement efforts. 3. Pulling and improving on this data metric will help your overall MIPS score if you choose it as one of your Quality Measures. 9

10 Transformation Plan Walkthrough Transformation Goals: This section asks you to rank these five areas of interest. Your evaluation should reflect your interest level in each of these initiatives. Please rate them from 1 (most interest) to 5 (least interest) You can only use each number one time. You will note that these measures once again coincide with either the clinical measures or the Practice Assessment Tool. It is important to remember that each step in this process is a component of the initiative to transform clinical practice. 10

11 Transformation Plan Walkthrough Vision for Transformation: Your TCPi participation is framed by your practice s vision statement. A vision statement is an organizational dream state or goal. It provides a roadmap or specific reason for being and an ideal of what you are working towards. Example: The driving purpose of my work is to achieve better patient health outcomes through increasing collaboration and education. 11
