9 Training Agreement The fllwing training agreement needs t be cmpleted by the student, a parent r guardian f the student, training facility supervisr, and teacher. Training Site/Supervisr 1. The supervisr at the training site will cmplete evaluatins as scheduled by the teacher. 2. If the student is emplyed, the student emplyment will be within the prvisins f all state and federal child labr laws and existing labr management agreements. The emplyer agrees t cmply with all WBL regulatins and if utilizing cperative educatin fllw all student-learner applicable state and federal regulatins, will prvide student trainees equal pprtunity emplyment and will nt discriminate n the basis f race, clr, natinal rigin, including limited English prficiency, sex r handicapping cnditins. 3. The term f agreement shuld be fr an agreed upn length. Either party shall have the right t terminate this agreement upn written ntice. 4. The emplyer/facility will prvide adequate staffing in the instructinal areas s that n student will be expected t perfrm duties withut supervisin. 5. The emplyer/facility will prvide an pprtunity fr the student t keep up t date with plicies and new technlgy by ntifying the schl f changes in plicies and technlgy. 6. The emplyer/facility will nt emply r cntract fr the services f students r faculty members during established schl hurs. 7. T assure that the emplyer/facility has sufficient resurces t meet its bligatins under the agreement, bth parties shall cnfer prir t the start f each semester regarding the students wh will participate in the prgram at the facility and their apprximate schedule fr the semester. 8. It is the respnsibility f the student, parent and the emplyer/facility t ntify the crdinatr in writing f any accident that ccurred while at the training site. The crdinatr will visit and/r cntact the training site at regular intervals t assess the student learner, t discuss the student s prgress and find ut what related instructin is needed. 2. Safety rientatin and prcedures instructin pertaining t the training site will be supplied by the emplyer. General Wrk Based Learning instructins t the student will be cvered in the related class by the teacher. 3. The emplyer and the schl will prvide instructin and experience at the training site and in the classrm. 4. The schl assumes full respnsibility fr ffering an accredited educatin prgram. 5. The instructr will plan the schedule and assist with assigning students t training sites. 6. Students and faculty will abide by existing rules and regulatins f the facility insfar as they may pertain t their activities while in the facilities building. The facility supervisr and/r crdinatr may remve students immediately that are believed t nt be cnducting their behavir in the best interest f the safety f themselves r thers. 7. The schl will require students and faculty t maintain current health recrds and immunizatins. 10 Student Agreement: Students will 1. Cmplete designated instructinal time and curriculum while maintaining academic grades, attendance and graduatin requirements t prgress t wrk based learning experience. 2. Cmplete the designated minimum hurs f supervised training at assigned facility as directed by the schl prgram. 3. Maintain minimum dress standards determined by yur training site and/r prgram crdinatr fr prfessinalism and safety expectatins. 4. Perfrm skills at the training facility that are apprpriate and within the training instructins. 5. Cntact the fllwing prir t their scheduled time: a) the schl (parent r guardian) b) training facility (student) c) instructr (parent r student) if they will be absent frm r tardy t the schl r training facility fr any reasn. 6. Prvide his/her wn transprtatin t and frm the training site. 7. Remain at the training site unless a request t transfer is apprved by the teacher-crdinatr. All training sites must be apprved by the teacher-crdinatr. 8. Be remved frm the prgram r prevented frm returning t an advanced class if students are released frm the training site by the facility fr a justified reasn. 9. Reprt a wrksite related injury t the crdinatr by the end f the next schl day. 10. Fllw the prvisins f the state and federal child labr laws. 11. Nt be required, r recmmended, t drive t, r reprt t, any internship site during any part f a day cvered by an annunced schl delay r schl cancellatin due t extreme inclement weather. Hwever, we will nt interfere with individual student decisins t drive t, r reprt t, any internship site during any such perid if the student, the student s parents (if the student is under the age f 18) and the student s internship site supervisr/mentr cnclude that such travel can take place withut undue risk t student safety.
