2 In order to participate in field trips, social events, and educational events, you must sign the contract below. Contract for Waiver of Legal Rights Notice: This release form is a contract with legal consequences. Please read it carefully before you sign. If you do not understand it fully, seek the advice of an attorney. Description of activity: All functions of the Flagstaff Home Educators I hereby waive, release and discharge myself, my heirs, executors, administrators, legal representatives, assigns and successors in interest, of any and all rights and claims which I may have or accrue against Flagstaff Home Educators, its members, board, officials, agents or officers, for any and all damages which may be sustained by me directly or indirectly in connection with, or arising out of, my participation or association with the group (Flagstaff Home Educators) or travel to or return from any meeting, field trip, social event, or educational event. I release Flagstaff Home Educators from liability for any and all claims of negligence I may have or assert against them individually or collectively, with the exception of willful and wanton negligence. I understand and agree that all these terms of waiver of legal rights apply to my children or any children for which I have agreed to be responsible. I have read and fully understand and agree to the above contract. Furthermore, I have read the Flagstaff Home Educators Policies and Guidelines and the Statement of Faith. 3 POLICIES AND GUIDELINES I. PURPOSE Flagstaff Home Educators is a Christian support group in Flagstaff, AZ for families who have chosen to educate their children without government intervention, oversight, or control. The main objectives are to provide family activities, informative parent meetings, & social/academic events for children. Parents are encouraged to be informed of & adhere to Arizona laws regarding homeschooling & to support those organizations which protect the right to home educate in AZ (i.e. AFHE & HSLDA) II. MEMBERSHIP A. A member family must be educating their children at home. B. Each family must fill out a Member Profile form. C. Dues are $30 for the year (June through May) & are not prorated if you join during the year. D. Each family is required to take a job when they register. III. EVENTS A. If you are planning an event, check the FHE website calendar for any previously scheduled event that might conflict with your activity. If the date you want is clear, select the option to post a new event on the website calendar. A form will open - fill it out with your event info. Click on Submit when you are finished. Upon receiving the event info, the FHE Membership/Events Coordinator will approve/post it promptly. B. FHE academic competitions (i.e. spelling bee, geography bee, Lifesmarts, science fair, etc.) are open to students who are homeschooling according to AZ homeschool law A.R.S

4 IV. COMMUNICATION A. The FHE Website is available for discussions, questions, notifying the group of upcoming events, advertising lessons & homebased businesses, advertising curriculum for sale, etc. V. BOARD OF DIRECTORS A. Board positions are offered by invitation only. Anyone interested in serving on the Board may make their wished known to the current Board. They must be willing to serve for a minimum of one year (preferably two years), be a professing Christian, sign the statement of faith, & be homeschooling/committed to homeschooling. B. Board members need to attend board meetings & as many other FHE meetings/ events as possible. C. The Board has the responsibility & authority to establish & revise policies, to approve or deny membership, jobs, activities & anything else pertinent to the operation of FHE. VI. STATEMENT OF FAITH We believe: In the Authority of Scripture - the Bible is God s inspired, infallible, & inerrant written Word. In one all-powerful, sovereign God, who is present in three distinct, yet united, persons: God the Father, God the Son, & God the Holy Spirit. God created all things in the heavens & the earth. All human life is sacred & created by God in His image. Salvation if the free gift of God, given by His grace & mercy to all who believe in Jesus Christ as their Lord & Savior. Revised January 2018
