1 SLATE OF CANDIDATES Ballots will be distributed to eligible voting members of FSHP on August 10, Voting will end on August 24, The President Elect will serve three years, one as President Elect, then as President and the last year Immediate Past President. All three terms they will be voting members of the FSHP Executive Committee. Three Board Elects will be elected. Elects will serve one year as a non voting member of the Board and two years as voting members of the Board of Directors. FOR PRESIDENT ELECT: Sandy J. Estrada, Pharm.D., BCPS (AQ ID) Following graduation from the University of Toledo Pharmacy School, Dr. Estrada completed a residency in critical care/infectious diseases and a fellowship in infectious diseases, also at the University of Toledo. She is the infectious diseases clinical specialist for Lee Memorial Health System, cochair of the antimicrobial stewardship committee, provides care to patients on the ID consult and HIV services and provides education to pharmacy and medical staff. She serves as preceptor for pharmacy students and PGY 1 pharmacy residents and is the program director for the PGY 2 Infectious Diseases Residency at Lee Health. Dr. Estrada is an active member of multiple professional pharmacy organizations including ACCP, ASHP, FSHP and SIDP. She is the former Secretary/Treasurer of SIDP, and worked with ASHP to draft new standards for PGY2 pharmacy residency programs in infectious diseases. She is also a past member of the FSHP Board of Directors and has chaired the Administrative Affairs Council. Personal Statement: It is an exciting time to be a pharmacist in the state of Florida and I am excited and passionate about the opportunity to be involved in shaping the future of FSHP, representing the needs and concerns of my colleagues. I am honored to be nominated and would be delighted to have the opportunity to lead FSHP. Dale Spears, RPh Dale received his pharmacy education from the University of Cincinnati. He has four sons, one a coach with the Boston Red Sox, one that is a pharmacist, one that is a physical trainer, and one is autistic. Dale has been a leader in the community where he has been a School Advisory Council Chairperson at an elementary school and a high school, where he made two principals Principal of the Year. Dale s leadership continued to excel within the profession of pharmacy. His professional career started at St. Elizabeth Medical Center in Northern

2 Kentucky. Although not computerized yet, St. Elizabeth Hospital was very progressive with clinical practice. With Antibiotic Stewardship not formally being implemented yet, they were involved with 3 day stop orders or re new antibiotics. From there, Dale decided to make a career enhancement and move to Florida. He started as Clinical Pharmacist and was promoted to Director of Pharmacy at Fawcett Memorial Hospital. Dale developed their first Medication Formulary and established drug utilization review. Dale went on to be a Clinical Pharmacist at then Lee Memorial Hospital in Fort Myers. He was responsible for removing six contingencies from the Joint Commission and make Lee Memorial a Hospital with Distinction. While at Lee Memorial, he wrote an article published in the FSHP Award winning journal on Reporting Adverse Drug Reactions. After Dale s home was destroyed by Hurricane Charlie, he moved to Tampa working at St. Joseph s Hospital. While at St. Joseph s Hospital, Dale was involved with two different presentations on Vancomycin Dosing and one led to a presentation at the 15 th International Congress on Infectious Diseases in Bangkok, Thailand. Dale s previous involvement with the Society includes President of the Southern Gulf Society, FSHP Board of Directors, Legal and Regulatory Affairs Council, and currently on the Professional Affairs Council. Personal Statement: I am seeking your support in my quest to SERVE you in the position of President Elect of FSHP. I believe one must serve before they can ever lead. My goal will be to address your highest priority needs and as a member of the governing board I will actively listen and seek out your opinions and ideas. I realize I cannot do this alone; your vote will allow me the ability to work with and for you. WHY, because I am honest, empathetic and I have a passion for Pharmacy. FOR BOARD OF DIRECTORS: Xi Liu DeRyke, Pharm.D., FCCM Xi Liu DeRyke, PharmD, FCCM, is currently a clinical specialist at Orlando Regional Medical. Dr. Liu DeRyke received her Pharm.D. from the University of Southern California. She completed her pharmacy practice residency at Florida Hospital South and practiced as a clinical pharmacist at Cedar Sinai Medical Center for 2 years. She then completed her specialty residency in critical care at Detroit Receiving Medical Center, and a two year clinical outcome fellowship in neurocritical care at Wayne State University. In addition to her daily practice with the trauma service, she is passionate about clinical outcome driven research and teaching residents/students. She has authored many peer reviewed articles, book chapters, and research abstracts, served as an active peer reviewer for several journals, and presented both nationally and locally. She believes strongly about giving back to the profession, and has displayed significant leadership roles throughout her career in many different professional organizations including Society of Critical Care Medicine (SCCM), Neurocritical Care Society (NCS), and served on various committees of Florida Society Health System Pharmacists (FSHP) (i.e. program committee, membership committee). Although she temporarily left FSHP due to relocation to another state, she immediately reached out to the society for opportunities to contribute upon her return to Florida. Dr. Liu DeRyke has been an active contributing member of FSHP, and she looks forward to serve the society and the profession in a larger capacity such as board of director.

3 Personal Statement: I believe it is only through sharing of the knowledge, giving back to the profession by teaching and mentoring the next generation of pharmacists, and be a voice for the profession, we continue to advance practice and improve our pharmacy profession. Patricia Louzon Lynch, Pharm.D., BCPS Dr. Patricia Lynch received her Doctor of Pharmacy from the University of Florida College of Pharmacy in She completed a PGY 1 Pharmacy Residency and a PGY 2 in Critical Care at Orlando Regional Medical Center in Orlando, FL. Dr. Lynch joined Florida Hospital Orlando in 2009 as a Critical Care Clinical Specialist and now leads the Critical Care/Emergency Department Team as the Clinical Manager. She is also the Program Director for the Critical Care PGY2 Residency. Research by Dr. Lynch has been published in Critical Care Medicine, Annals of Pharmacotherapy, American Journal of Infection Control and Hospital Pharmacy. In 2015, she led her Critical Care team to an ASHP Best Practice Award for their ICU sedation management. Dr. Lynch is also a Clinical Assistant Professor at the University of Florida College of pharmacy, where she precepts, provides lectures, has facilitated a Critical Care Elective and was named Preceptor of the Year in Dr. Lynch has been involved with FSHP for the past 14 years, serving as a Delegate for 10 years, an Educational Affairs Council member and Annual Meeting program planner for 6 years and on the Committee on Nominations. She has also served in Regional leadership roles for 5 years as President, President elect, Past President and Secretary of the Central Florida Chapter (CFSHP). She is also an active member of Society of Critical Care Medicine (SCCM) and American Society of Health System Pharmacists (ASHP). Personal Statement: Over the past 14 years, I have enjoyed working with all society members and serving FSHP in many leadership roles. I believe my experience with leadership through FSHP and my role as a pharmacy clinician and manager will position me well to serve the state society. I am passionate about society involvement and advancing the pharmacy profession and I would consider it an honor to represent our members on the Board of Directors. Susan Mitchelson, BS Pharm, CPh Susan Mitchelson has more than 24 years of experience as a pharmacist, leader and coach. She started her career with CVS where she earned a reputation as a store turnaround manager, found her passion for coaching and mentoring students, and earned a district Paragon Award, the highest honor a CVS employee can achieve. Upon relocating to Orlando, Susan embraced a new challenge, becoming the leading pharmacist in the treatment of Hepatitis C for patients encompassing all of Florida and building a multi million dollar revenue channel for CVS ProCare. Susan entered hospital pharmacy in 2003 and became Pharmacy Supervisor at Lee Memorial Hospital in 2007 (now Lee Health) and subsequently earned a consultant pharmacist designation.

4 In addition to leading and working with a great team of pharmacists, technicians and residents, Susan continues to follow her passion for mentoring students. She serves as resident advisor, adjunct faculty, preceptor, and career coach for several schools including University of Florida, NOVA, LECOM and St. Louis College of Pharmacy. Susan has been elected as the secretary of the Southern Gulf Regional chapter of FSHP for the past 2 years and has served as a representative at the House of Delegates and on the Membership Affairs Council. She is also a member of the America Society of Health System Pharmacists. Originally from upstate New York and a graduate of Albany College of Pharmacy, Susan has found happiness in southwest Florida where she enjoys spending time with her family and friends. Personal Statement: I am honored to be slated as a candidate for the Board of Directors. One of my goals as a pharmacist is to give back to a profession that has been so good to me. A few years ago, I reevaluated what being a member of FSHP meant to me and the answer was to be actively involved. I look forward to learning from and hopefully contributing to the FSHP Board. Kelly Owens, Pharm.D., BCPS, BCCCP Dr Owens is a Clinical Pharmacy Specialist in Critical Care and Emergency Medicine at Dr. P. Phillips Hospital in Orlando, FL. In 2015, she was awarded Dr. P. Phillips Hospital Beginning Practitioner of the Year. Dr. Owens earned her Doctorate of Pharmacy degree from South Carolina College of Pharmacy in 2012 and completed her post graduate training at Greenville Health System in Greenville, SC specializing in critical care. During her career in South Carolina, she served as adjunct instructor for South Carolina College of Pharmacy. In Florida, she maintains an active role with students as a preceptor and career coach for University of Florida College of Pharmacy. Dr. Owens has served Florida Society of Health System Pharmacists as Co Chair of Education Affairs Council and as a delegate to the House of Delegates. Other previous service includes South Carolina Society of Health System Pharmacist Education and Planning Committee, and active membership in American Society of Health System Pharmacists and American College of Clinical Pharmacy. Personal Statement: I am honored to be nominated for Board of Directors. I believe FSHP is positioned to inspire, innovate and integrate our profession into the future of health care and I welcome the opportunity to serve FSHP members in our journey forward together. Calvin Tucker, Pharm.D., BCPS, BCCCP Dr. Tucker is the Clinical Manager of Pharmacy Services at St. Vincent s Medical Center Riverside in Jacksonville, FL. Originally from Jacksonville; he obtained his doctorate in pharmacy from the University of Florida in Dr. Tucker completed a PGY 1 Pharmacy Practice Residency at UF Health in Jacksonville, FL. To focus his goals and interests, he traveled to Miami, FL to complete a PGY 2 in Critical Care at Jackson Memorial Hospital. Dr. Tucker is also board certified in pharmacotherapy and critical care pharmacy.

5 In addition to working as a clinical manager, Dr. Tucker is Clinical Assistant Professor at the University Florida College of Pharmacy serving a course facilitator for second year and fourth year pharmacy students. Dr. Tucker currently serves as the Community Service Chair and previously served as President, of the Northeast Region of the Florida Society of Health System Pharmacist. He also served on various councils within FSHP such as professional affairs council, education council and membership affairs council. In addition Dr. Tucker is involved with the communications committee of the clinical pharmacy and pharmacology section of the Society of Critical Care Medicine. Dr. Tucker has given several presentations and published articles in the areas of critical care and infectious diseases. Personal Statement: It would be an honor to serve on the FSHP Board of Directors. My involvement at the regional and state level has revealed the pivotal role professional organizations, particularly FSHP, plays in shaping the future of pharmacy. The opportunity to serve on Board of Directors will allow for continued professional growth, further my involvement within the organization and contribution to initiatives critical to the progression of our profession. FOR TECHNICIAN DIRECTOR: Sandra Barnes, BS, RPhT, CPhT I am originally from Jacksonville, FL. I entered into pharmacy at the age of 15 in 1983 where I worked as a drug clerk at Eckerd Drugs. I have worked in many facets of pharmacy, the majority being in hospital pharmacy, where I worked most often as an IV technician. I completed my Bachelor s Degree in Biology at University of North Florida in Jacksonville in December 2000 and transitioned into teaching. I started teaching pharmacy technicians in 2004 thru I have since returned to the field to broaden my knowledge base and work as a medication reconciliation technician in the Emergency Room at St. Vincent s Hospital Riverside Jacksonville, FL. I joined the FSHP in 1993 when there wasn t a technician division. I have worked on many of the councils through the years; mainly the Educational Affairs Council and Legal and Regulatory Affairs Council. I assisted our group in getting Technician Registration legislation passed and mandated for the pharmacy technician in the state of Florida. I was the recipient of the FSHP Pharmacy Technician of the Year in I have been a member of the House of Delegates for the organization for a great number of years and have worked closely with getting many resolutions passed including the resolution that allows pharmacy technicians to sit on the Board of Directors. Personal Statement: My passion is my drive to see a tier system in the pharmacy profession as there is with the profession of medicine. I plan to assist in moving the resolution that just passed for a pharmacy technician to sit on the Board of Pharmacy forward. I also plan on getting changes made to registration to include certification to mandate the ability to show our clinical competencies. I also plan to work on getting rules changed with the Board of Pharmacy to create a standardized pharmacy technician training program that could limit some of the abilities of the employer based training programs especially when not all facets of the field are taught. My passion is to move and grow the profession of a pharmacy technician as our roles are evolving along with the pharmacist s roles in our profession.
