Complete the replacement application for the specific state. All requests require a signature for verification. If you submit without the signature of the person on the DL/ID, the request will not be processed. In the remarks section, the applicant should provide contact information, including phone, mobile phone or pager, if possible. Also include the reason for being out of state as evacuation due to Hurricane Katrina.

2 Tennessee Department of Human Services The Tennessee Department of Human Services (DHS) and TEMA are coordinating assistance for Katrina refugees from Louisiana, Mississippi, and Alabama. Hurricane Refugees may be entitled to food stamps and other federal benefits under the National Refugee Policy. If you are seeking these benefits, please go to the nearest DHS office or work through local aid workers and TEMA officials. U.S. Postal Service The U.S. Postal Service is reconnecting displaced and evacuated Hurricane Katrina victims with their mail. Hurricane victims in areas without telephone service or internet access should go to the nearest post office, complete a change-of-address form, and submit it to a postal retail associate at the counter or mail it. Only animals such as Seeing Eye dogs are permitted. If you live in an evacuation zone and plan to go where your pet will not be welcome, make arrangements for a safe place for your pet prior to a hurricane.

6 Call your veterinarian to find out where you can board your pet if a hurricane threatens. Contact hotels and motels in non-evacuation areas to see if they will allow you and your pet to stay with them during a storm. Make sure you have supplies for your pets for 2 weeks.
