2 CPhA Honorary Life Award The CPhA Honorary Life Award recognizes outstanding service to pharmacy, CPhA and the profession nationally. The award may be conferred on any pharmacist, who is or has been a CPhA Individual Associate or Individual Affiliate, or non-pharmacists and non-residents of Canada who have contributed to CPhA or the profession nationally. The number of CPhA Honorary Life Awards granted in one year is not limited; if there are no qualified candidates nominated, the award will not be granted. CPhA Honorary Life award winners shall be granted lifetime affiliation to CPhA. 3 CPhA Meritorious Service Award The CPhA Meritorious Service Award is offered in recognition of service to the pharmacy profession such that it brings credit and prestige to the profession in Canada or internationally. The award may be awarded posthumously. The award may be conferred on anyone who is or has been a CPhA Individual Associate or Individual Affiliate. The number of CPhA Meritorious Service Awards granted in one year is not limited; if there are no qualified candidates nominated, the award will not be granted. Contribution to a national or international pharmacy organization through service on the Board or committee work Active in community service as a pharmacist Recognition and/or honour related to pharmacy activities (provincial, national, international) To nominate based on international contribution, evidence of a prominent international leadership role within pharmacy, either through an individual initiative or within an organizational framework (volunteer or workplace), which allows the nominee to make an international contribution in their sphere of activity resulting in positive profile brought to the profession within Canada The successful recipient will receive, for the year in which the award is presented: Recent recipients of the CPhA Meritorious Service Award: 2017 Paula MacNeil 2015 Dennis Gorecki 2012 Ray Murphy 2011 Ronald Guse Régis Vaillancourt Barbara Wells 2010 Fred Martin 2007 Melanie Rantucci

4 CPhA New Practitioner Award The CPhA New Practitioner Award recognizes a pharmacist who has been in practice not more than five years, has demonstrated a commitment to pharmacy at the undergraduate level and has continued to promote the profession. The award may be conferred on any CPhA Individual Associate or Individual Affiliate who has been licensed to practice pharmacy in a province or territory of Canada not more than five years. Only one CPhA New Practitioner Award may be granted in a given year; if there are no qualified candidates nominated, the award will not be granted. 5 CPhA Patient Care Achievement Award Sponsored by Green Shield Canada Foundation in honour of Vernon Chiles The CPhA Patient Care Achievement Award recognizes a pharmacist who has demonstrated excellence in a practice setting by exhibiting outstanding innovation and leadership, establishing a specialty practice, or going above and beyond in the ongoing education of patients on wellness, healthy lifestyles and disease prevention. The award may be conferred on any CPhA Individual Associate or Individual Affiliate. Only one CPhA Patient Care Achievement Award may be granted in a given year; if there are no qualified candidates in a particular year, the award will not be granted. Unique and/or creative ongoing wellness promotion, initiated or driven by the nominee Innovative pharmacy practice, initiated or driven by the nominee, that is applicable or relevant to other practice sites and that contributes to public health Community work related to health promotion activities or specialty practice Involvement in educating the public and health professionals Development and implementation of clinical tools and/or programs Advanced training or certification The successful recipient will receive, for the year in which the award is presented: This award is new for 2018, combining the former CPhA Patient Care Achievement Awards for Health Promotion, Innovation and Specialty Practice.

6 CPhA Certificate of Recognition The CPhA Certificate of Recognition is conferred on pharmacy groups (excluding pharmacies) or professional organizations, and in exceptional cases, an individual, to recognize outstanding service or contribution to the profession and the community. Anniversaries (50-year increments) or special occasions may be acknowledged. The number of CPhA Certificates of Recognition granted in one year is not limited; if there are no qualified candidates identified in a particular year, the certificate will not be granted.
