Here, you can find services that are covered under your North Shore-LIJ Employee Plan as Tier 1 in the Clinical Integration Network IPA, LLC (CIIPA) also known as in-system. Accessing providers in Tier 1 assures you will reduce your out-of-pocket expenses. Please check the table of contents on the next page to see what services are covered by our Employee Plan. Locations and contact information are also included for the listed facilities. How to Find a Tier 1 Provider on the CIIPA Website: Look up a provider using the CIIPA website at Once you have arrived at the CIIPA website, click I AM A MEMBER. 3 Next, you will have the option to select the North Shore-LIJ Employee Health Plan: You will then have the option to search for a healthcare provider by name, or by category, specialty, and location. It s important when searching for a provider by Specialty type that you enter the provider category and the provider sub-specialty under Select a Specialty. This assures you will find the provider with the clinical expertise you need. Provider Nominations If you would like to nominate an Obstetrics and Gynecology, Pediatrician, Internal Medicine or Family Practice (PCPs) provider for CIIPA participation, please us the provider s first and last name, group name, office location, primary specialty, phone number, and fax number to CIIPA@nshs.edu. 6 DIABETES WELLNESS AND CHRONIC DISEASE MANAGEMENT PROGRAMS GOALS ( ) The mission of the North Shore-LIJ Diabetes Wellness and Chronic Disease Management Programs is to provide resources to those living with chronic disease to help them manage their condition and maintain a healthy lifestyle. The programs support patients, families and health care providers by offering the latest treatment and educational resources, and encourage community involvement. We have locations in Nassau, Suffolk and Queens. Our diabetes education program is accredited by the American Association of Diabetes Educators. We care for people living with Type 1 diabetes, Type 2 diabetes and gestational diabetes. The Chronic Disease Management Program also offers individualized medical nutrition therapy.
