1 St. John s PGY-1 Residency Program Preceptors Jared Anderson, PharmD, MS Pharmacy Operations Manager Informatics Preceptor Jared received his Doctorate of Pharmacy degree from the University of Minnesota College of Pharmacy in After graduation, he completed a PGY1/PGY2 administrative pharmacy practice residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin. During this time, he also earned a Master s degree in Health-System Pharmacy Administration at the University of Wisconsin. His areas of interest include pharmacy workflow and automation optimization, technician development and pharmacy informatics. He is a member of MSHP, ASHP, ACCP, USATF and serves on the ASHP Section Advisory Group on Pharmacy Operations and Automation. He currently serves on the St. John's Operations Engagement Committee, Pharmacist Scheduling Committee, HealthEast Pharmacy Operations Council and Pharmacy Clinical Operations Informatics (P-COIN) Committee. Outside of pharmacy, he enjoys running, cycling and camping with his family. Kristen Draz, PharmD, BCPS Clinical Pharmacy Coordinator- Ambulatory Anticoagulation Clinic Preceptor Kristen earned her PharmD from Drake University in 2010 and completed a PGY1 Pharmacy Practice Residency with HealthEast Care System. She is board certified in pharmacotherapy (BCPS). Kristen s clinical interests include direct oral anticoagulants, peri-procedural anticoagulation, drug interactions, infectious disease, and diabetes care. Kristen is a member of the HealthEast Anticoagulation Committee, HealthEast Pharmacy Practice Committee, the Minnesota College of Clinical Pharmacy (MCCP), and the Minnesota Pharmacists Association (MPhA). Paul Forsberg, PharmD, BCOP Lean Pharmacy Preceptor Paul received his PharmD from the University of Montana. His focused areas of interest include oncology, clinical pharmacy leadership, Lean, and pharmacy finance. Paul is the Vice President of the Upper Midwest Oncology Education Network and a current member of HOPA. He serves on various committees including Oncology Shared Nursing Leadership Council, P&T Committee, Pharmacy Clinical Operations Informatics (PCOIN) Committee, St. John s Residency Advisory Committee, St. John s Pharmacist Scheduling Committee, and the Order Set Oversight Committee. He currently serves as a preceptor for the Lean Pharmacy Residency Rotation.

2 Candace Globa, PharmD Oncology Preceptor Candace received her PharmD from Creighton University campus based program in Candace s interests are oncology, critical care and infectious disease. Candace serves as the primary preceptor for the pharmacy students during their oncology APPE patient care rotation. Candace serves as the primary preceptor for residents for the oncology elective rotation. Candace is a member of HOPA and the St. John s Residency Advisory Committee. Katie Kline, PharmD, BCACP Medication Management (MTM) Pharmacist MTM Preceptor Katie received her PharmD degree from the University of Minnesota College of Pharmacy, Twin Cities and completed a PGY1 Pharmacy Practice Residency with HealthEast Care System. She is a Board Certified Ambulatory Care Pharmacist (BCACP) and practices at the HealthEast Woodbury and Tamarack Clinics. Katie is an active member of several local and national pharmacy organizations, including the American College of Clinical Pharmacy (ACCP), Minnesota College of Clinical Pharmacy (MCCP), and Minnesota Pharmacists Association (MPhA). Ondrea Levos, PharmD Anticoagulation Clinic Preceptor Ondrea received her PharmD degree from North Dakota State University. She completed a residency in Nephrology at Hennepin County Medical Center in Minneapolis, Minnesota. Ondrea is the Pharmacy Clinical Manager for Specialty Care in Ambulatory Care at HealthEast and manages the HealthEast Anticoagulation Clinic for Primary Care. Ondrea serves on several committees including Primary Care Clinical Practice Group, P&T Committee, HealthEast System Anticoagulation, Medication Safety, Clinical Pharmacy Group, System Order Set Oversight Committee, System Policy Review Committee, St. John s Residency Advisory Committee and HealthEast Residency Oversight Committee. Stacy Livingston, PharmD Pharmacy Operations Manager Medication Safety Preceptor Stacy received her degree from the University of Iowa College of Pharmacy in Iowa City, Iowa. She completed a PGY1/PGY2 residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin where she focused her training in both clinical and operational practices. Stacy joined the St. Joseph s Hospital team in July 2014 and her focus is on drug distribution, sterile product preparation, quality improvement, pharmacy automation/technology and medication safety. She currently oversees the central pharmacist team, the pharmacy technicians and the pharmacy internship program at St. Joseph s Hospital. Stacy is a member of the Minnesota Society of Health-System Pharmacists (MSHP), the American College of Clinical Pharmacy (ACCP) and the American Society of Health-System Pharmacists (ASHP).

3 Pamela Motzko, PharmD, BCPS, Residency Program Coordinator Emergency Medicine Preceptor Pam received her BS and PharmD degrees from North Dakota State University. She has worked in multiple areas of hospital pharmacy practice, including ICU, transplant, oncology, pediatrics, and OR. After leaving a management position, she returned to clinical practice and obtained BCPS certification. She primarily applies her experiences as a member of the Critical Care team. As the ED preceptor, she is focused on patient safety, medication reconciliation, and expanding the role of the pharmacist in this area. She is a member of the Pharmacy Resident Advisory Committee, the Pharmacist Scheduling Committee and the Pharmacy Operations and Engagement Committee (OPEN). Brandon Ordway, PharmD, MS Director of Pharmacy, East Region Practice Management and Leadership Preceptor Brandon received both his PharmD and MS degrees from the University of Wisconsin. He completed a twoyear residency in Health System Pharmacy Administration and Leadership at the University of Wisconsin Hospital and Clinics. He is an active member of both the MSHP and ASHP. He is also a preceptor for the University of Minnesota College of Pharmacy with an APPE leadership rotation. His practice interests include pharmacy automation and technology, quality improvement of complex systems, regulatory compliance, medication reconciliation, and personnel training and development. Megan Sandgren, PharmD, MS, BCPS Project Preceptor Megan received her PharmD degree from North Dakota State University in She completed a PGY1&2 Health System Pharmacy Administration residency at the University of Minnesota Medical Center/Fairview Pharmacy Services and received her Master of Science in Social and Administrative Pharmacy from the University of Minnesota in At HealthEast, Megan assists in the clinical oversight of pharmacy services in the medicine, surgery, and OB areas and serves on various committees including the P&T Committee, Medication Reconciliation Workgroup, Obstetrics/Gynecology Council, and Orthopedic Quality Council. She is also an active member of the Minnesota Pharmacists Association (MPhA), MSHP, and ASHP. Megan s practice interests include internal medicine, transitions of care, metric and outcome reporting, and medication safety. Marit Lindquist, PharmD, DAAPM Clinical Pharmacy Pain Specialist Pain Management Preceptor Marit received her PharmD degree from the University of Minnesota in She completed a PGY1 residency with HealthEast (residents were not assigned to a certain hospital in , but instead floated throughout the system throughout the year). Marit is a member of the Academy of Integrative Pain Management, as well as MSHP and ASHP. She is a pain management specialist within the HealthEast Acute Care Pain Management Team. Marit has specialized in Acute and Chronic pain management for most of her career.

4 Shana Steinbeck, PharmD, BCPS Internal Medicine Preceptor Shana received her PharmD from the University of Minnesota College of Pharmacy in She completed a PGY1 residency with HealthEast Care System St. Joseph s Hospital in 2015 and joined St. John s pharmacy as a staff pharmacist following her residency. Shana s interests are in internal medicine, infectious disease, cardiology and transitions of care. She is also the acute care preceptor for APPE students from the University of Minnesota College of Pharmacy. Shana is a member of MCCP and St. John s Residency Advisory Committee. Jennifer Sutton, PharmD, BCPS Critical Care Preceptor; Staffing Preceptor Jenni received her PharmD from the University of Minnesota, Duluth College of Pharmacy in 2011 and obtained board certification in Pharmacotherapy in She has been a HealthEast employee since 2002 and currently serves on the Critical Care team. Jenni has been deeply involved in the medication reconciliation program throughout HealthEast and has served on several med rec committees including a stint as a Med Rec Subject Matter Expert during the development and implementation of Epic. She is currently the primary preceptor for the critical care PGY-1 residency rotation. She serves as Co-Chair of the HealthEast Med Rec Work Group and is a member of the St. John s Residency Advisory Committee, MPhA, MCCP, and Phi Delta Chi Pharmacy Fraternity. Jenni's areas of interest include critical care, infectious disease, and transitions of care. Natasha Thoner, PharmD, BCPS Cardiology Preceptor Natasha received her PharmD degree from the University of Minnesota, Twin Cities campus. She completed a PGY1 residency with HealthEast Care System in Natasha obtained her board certification in pharmacotherapy in Natasha s areas of interest include critical care, infectious disease, and cardiology. Natasha is a member of MSHP and the St. John s Residency Advisory Committee. Rebecca Zeccardi, PharmD, BCPS Clinical Pharmacy Coordinator, Residency Program Director Project Preceptor Rebecca received her PharmD degree from the University of Minnesota College of Pharmacy in Duluth, MN and completed a PGY1 Pharmacy Practice Residency with HealthEast Care System. She is Board Certified in Pharmacotherapy (BCPS) and is a member of the American Society of Health System Pharmacists (ASHP), Minnesota Society of Health System Pharmacists (MSHP), Pharmacy and Therapeutics Committee, Clinical Pharmacy Committee, and chair of the HealthEast Residency Oversight Committee and the St. John s Residency Advisory Committee. Becky s serves as the pharmacist representative on the HealthEast Neuroscience Clinical Council, Emergency Medicine Clinical Council, Spine Clinical Council, Stroke Committee, and Sedation Committee and is also interested in critical care.

5 Kara Webber, PharmD, MHA Orientation/Staffing Preceptor Kara received her PharmD from the University of Minnesota College of Pharmacy in She completed a PGY1/PGY2 pharmacy administration residency at University of Washington Medicine in Seattle in During residency, she also completed her Masters in Health Administration at the University of Washington. Kara s interests are in critical care, infectious disease, regulatory compliance, and pharmacy operations. Kara is a member of MSHP (serves on Membership Committee), ASHP and the St. John s Residency Advisory Committee.
