3 HOST/MODERATOR Amalia Cochran, MD, FACS, FCCM: University of Utah Health Care Dr. Cochran is an Associate Professor of Surgery and an Adjunct Assistant Professor of Pediatrics at the University of Utah School of Medicine. She is also a general surgeon at University of Utah Health Care. Her clinical practice focuses on acute burn care, critical care in burns and burn reconstruction. Dr. Cochran has a particular interest in advancing the clinical care of frostbite patients and has contributed significantly to the clinical care and body of research in this area. She has completed a surgical education research fellowship and has won university-wide and national teaching awards. Dr. Cochran also holds a number of leadership roles outside of the University, including serving as the Vice Chair of the ACS Professional Association Political Action Committee Board of Directors and President of the ACS Utah Chapter. She received her medical degree from Texas A&M University Health Sciences Center. OPENING PRESENTER David B. Hoyt, MD, FACS: American College of Surgeons Dr. Hoyt is the Executive Director of the American College of Surgeons (ACS). In this role, Dr. Hoyt oversees nationally and internationally recognized educational and quality programs, including education and training for surgeons and surgery residents, and quality improvement programs including the Committee on Trauma, Commission on Cancer, National Surgical Quality Improvement Program (NSQIP), National Accreditation Program for Breast Centers and Bariatric Surgery Center Network. Dr. Hoyt is the former Medical Director of ACS s Trauma Programs and served as Executive Vice-Dean of the University of California, Irvine, School of Medicine; Chairman of the Department of Surgery; and the John E. Connolly Professor of Surgery at the University of California, Irvine. An international lecturer, Dr. Hoyt is the author of over 480 book chapters, journal articles and abstracts.

4 KEYNOTE SPEAKER Brent C. James, MD, MStat: Intermountain Healthcare Dr. James is the Chief Quality Officer and Executive Director of the Institute for Health Care Delivery Research at Intermountain Healthcare. He is known internationally for his work in clinical quality improvement, patient safety and the infrastructure that underlies successful improvement efforts. Through the Intermountain Advanced Training Program in Clinical Practice Improvement, he has trained more than 5,000 senior physician, nursing, and administrative executives from around the world in clinical management methods with proven improvement results. Dr. James is a member of the National Academy of Science s Institute of Medicine and holds faculty appointments at the University of Utah School of Medicine, Harvard School of Public Health and the University of Sydney, Australia, School of Public Health. Before coming to Intermountain, Dr. James was an Assistant Professor in the Department of Biostatistics at the Harvard School of Public Health. Modern Physician has named him among the 50 Most Influential Physician Executives in Healthcare and Modern Healthcare has named him among the 100 Most Powerful People in Healthcare and 25 Top Clinical Informaticists. Dr. James received his medical degree, master s degree in statistics, and bachelor s degree in computer science and medical biology from the University of Utah. His residency training was in general surgery and oncology.

5 PANELISTS (in order of appearance) Vivian S. Lee, MD, PhD, MBA: University of Utah Health Care Dr. Lee is Senior Vice-President for Health Sciences at the University of Utah, Dean of the University s School of Medicine and CEO of University of Utah Health Care. She is a radiologist by training and is currently principal investigator for three NIH R01 grants. She has served as Chair of the Medical Imaging NIH study section and is a Fellow and past President of the International Society for Magnetic Resonance in Medicine (ISMRM). Dr. Lee has authored over 150 peer-reviewed research publications and a textbook. In 2005, 2011 and 2012, Dr. Lee received the Outstanding Teacher Award from the International Society for Magnetic Resonance in Medicine. She received NYU s Orloff Award in 2001 for research and was among Crain s New York Business magazine s 40 under 40: New York s Rising Stars. She has been awarded the Chang-Lin Tien Education Leadership Award, Polytechnic-NYU Innovator s Award, Dynamic Achiever Award Overseas Chinese Association, and Pathfinder Award SLC Chamber of Commerce. After graduating from Harvard, Dr. Lee was awarded a Rhodes Scholarship to study at Oxford University, where she received a doctorate in medical engineering. She returned to Harvard Medical School to earn her medical degree with honors. She completed her residency at Duke, serving as Chief Resident, and trained as a fellow at NYU where she also completed her MBA and served as the inaugural Vice Dean for Science, Senior Vice- President and Chief Scientific Officer of New York University Medical Center. Charles Sorenson, MD, FACS: Intermountain Healthcare Dr. Sorenson is the President and Chief Executive Officer of Intermountain Healthcare and an Adjunct Professor of Surgery at the University of Utah. He helped create the Intermountain Medical Group, serving as the founding chair from 1994 to He then assumed his full time administrative duties with Intermountain and served as Executive Vice President and Chief Operating Officer of Intermountain Healthcare until Under Dr. Sorenson s leadership, Intermountain has implemented a disciplined, system-wide focus on best clinical practices aimed at producing measurably better outcomes for patients. Similar efforts are being applied to operational best practices aimed at increasing efficiency and holding down costs. His professional activities have included serving as President of the medical staff at LDS Hospital, chairing the Committee on Young Surgeons for ACS, and serving as President of the Utah Urologic Society and as Speaker of the House of Delegates of the Utah Medical Association. Dr. Sorenson received his medical degree from Cornell University Medical College.

6 PANELISTS (continued) E. Marc Mariani, MD: St. Mark s Hospital Dr. Mariani is an orthopaedic surgeon at St. Mark s Hospital. He has served as Chief of the Orthopaedic Division, Chairman of the Board at St. Marks, and President of the Utah State Orthopaedic Society. After receiving his medical degree at the University of Utah, Dr. Mariani pursued his orthopaedic training at the Mayo Clinic. Following his residency, where he served as Chief Resident Associate, he remained at Mayo on staff as an Assistant Professor at the Mayo Medical School. In 1989, Dr. Mariani returned to Salt Lake City to form the Salt Lake Orthopaedic Clinic where he practices to this day. Sara Hartsaw, MD, FACS: High Plains Surgical Associates, PC Dr. Hartsaw is a general surgeon and owner at High Plains Surgical Associates, PC in Gillette, WY, where she is known in the community for her expertise and quality surgical work. She is Chair of the Campbell County Memorial Hospital Board of Trustees, and provides surgical services at Campbell County Memorial Hospital, North East Wyoming Surgical Center and Powder River Surgical Center. In 2013, Dr. Hartsaw was also commissioned as a Lt. Commander in the United States Naval Reserve. She received her medical degree from Creighton University School of Medicine, and completed her residency in the Phoenix Integrated Surgical Residency Program based at Banner Good Samaritan Medical Center in Phoenix, AZ. Timothy B. Anderson: Jones Waldo PC, University of Utah Hospitals and Clinics Timothy B. Anderson is managing attorney of the St. George, Utah branch office of Jones Waldo PC, and leader of the firm s General Counsel Services and International Law Practice Groups. In addition to his emphasis in international commercial transactions and corporate law, he also practices in the area of health care law with a focus in select areas of regulatory compliance and governance. He served as a Trustee of the University of Utah from 2005 to 2013 and is currently a member of the Board of Trustees of University of Utah Hospitals and Clinics, co-chairing the hospital s Patient Safety and Value Committee and serving as Vice Chair of the Utah State Bar Health Care Law Section. Anderson received his bachelor s degree in political science from Southern Utah University and his law degree from J. Reuben Clark Law School at Brigham Young University.

7 PANELISTS (continued) Samuel R.G. Finlayson, MD, MPH, FACS: University of Utah School of Medicine Dr. Finlayson is a Professor and the Claudius Y. Gates, MD, and Catherine B. Gates Presidential Endowed Chair of the Department of Surgery at the University of Utah School of Medicine. He has developed a national reputation as a health services and outcomes researcher primarily focused on surgical access, quality and outcomes. He has a specific interest in the quality of surgical care delivery in rural versus urban settings in the United States, as well as low income and resource poor countries. Prior to his current role, Dr. Finlayson served as the Kessler Director of the Center for Surgery and Public Health, a joint initiative between the Brigham and Women s Hospital and Harvard Medical School. Prior to that, he spent a decade at Dartmouth Medical School, where he was Vice Chair of the Department of Surgery, Director of the Surgical Residency Program, and Associate Professor of Surgery and of Community and Family Medicine. Dr. Finlayson received his medical degree from Harvard Medical School and his master s degree in public health from the Harvard School of Public Health. He was trained in surgery at Massachusetts General Hospital, and completed a surgical outcomes research fellowship at Dartmouth Medical School. Mark J. Ott, MD, FACS: Intermountain Healthcare Dr. Ott is Medical Director for Surgical Services at Intermountain Healthcare, and Adjunct Associate Professor of Surgery at the University of Utah School of Medicine. He specializes in hepatobiliary and pancreatic tumors, sarcoma, complex upper gastrointestinal surgery and other rare tumors. His leadership interests center on recruiting physicians and surgeons to participate in clinical program building, outcomes analysis and research. Dr. Ott continues to lead corporate-wide initiatives to reduce the cost of surgical procedures by tens of millions of dollars per year. He has served as the physician champion for Intermountain Medical Center s participation in the National Surgical Quality Improvement Program (ACS NSQIP). He also has a strong interest in workplace wellness. Prior to being recruited to Intermountain Healthcare, he practiced surgical oncology at Massachusetts General Hospital and was a faculty member at the Harvard School of Medicine. Dr. Ott received his medical degree from the University of Utah School of Medicine. He completed his residency in general surgery and fellowship in surgical oncology at the Johns Hopkins University School of Medicine.
