She earned a BS degree in Nursing from SDSU, an MS in Nursing from the University of Portland, and a PhD in Nursing from the University of Nebraska Medical Center. She joined the faculty at SDSU in 1995, and previously served as the inaugural Associate Dean for Research in the College. As Dean, she represents the College as an executive leader within the University and she oversees all academic programs, research, and outreach activities. The College offers four nursing education degree programs (BS, MS, DNP and Ph.D.) and four certificate programs with eight specializations and multiple entry and program completion options. Programs are offered at four statewide delivery sites and online. Dean Fahrenwald maintains an active program of research in health promotion for rural and American Indian populations and she serves in national leadership roles in her area of expertise. She is recognized as a nursing and public health leader across the state, region and nation. In 2014, Dean Fahrenwald was inducted as a Fellow in the American Academy of Nursing.

3 Search Committee Member: Dan Hansen Office Phone: (605) Cell Phone: (605) Assistant Dean for Student Services Associate Professor of Pharmacy Practice College of Pharmacy and Allied Health Professions Dan earned both his B.S. and Pharm.D. from South Dakota State University. Upon graduation, he moved to Milwaukee, WI, where he worked as a pharmacy manager for Walgreens. Dan returned to SDSU and the College of Pharmacy and Allied Health Professions in 2007, when he was appointed Experiential Program Manager and an Assistant Professor of Pharmacy Practice. In 2010, Dan was appointed the Assistant Dean for Student Services. As the Assistant Dean, he is responsible for overseeing the recruitment and retention of students, the admissions process for the Pharm.D. and Medical Laboratory Science program, student conduct, scholarships, and academic support services. In Summer 2016, he also took over administrative responsibility for the College s Medical Laboratory Science program. Dan s primary teaching responsibilities include pharmacy calculations, communications, and over the counter medications. He also coordinates the College s freshmen introductory to pharmacy course, takes P4 students on an academic rotation, and teaches courses within the Fishback Honors College. Dan s scholarship has included projects focused on the scholarship of teaching and learning, admissions, student success, and patient counseling.

4 Search Committee Member: Jodi Heins Professor and Assistant Department Head Pharmacy Practice Office Phone: (605) Cell Phone: (605) College of Pharmacy and Allied Health Professions Jodi Heins is the Assistant Department Head in the Pharmacy Practice Department for the College of Pharmacy and Allied Health Profession and is in her sixth year in this role. Prior to this she had served in a shared faculty position with the VA Medical Center in Sioux Falls, SD as a clinical pharmacy specialist with both an ambulatory care practice and an inpatient practice for eighteen years. She earned her Pharm.D. from the University of Nebraska Medical Center College of Pharmacy and completed a Pharmacy Practice Residency at Mayo Medical Center in Rochester, Minnesota. As Assistant Department Head, she oversees the experiential components of the Pharm.D. curriculum and is a member of the Executive Team for the College. She is responsible for the Teaching Certificate offered by the College to pharmacy residents and preceptors across South Dakota. She also serves as the Residency Program Director for the PGY1 Community-based Pharmacy Residency.

5 Chris Kotschevar Pharmacy Student College of Pharmacy Cell Phone: (507) Address: Chris Kotschevar is a second-year pharmacy student at South Dakota State University. Chris completed two years of pre-pharmacy coursework at SDSU prior to starting in the professional program. Throughout his time at South Dakota State, Chris has been active in the Fishback Honors College, worked as an Orientation Leader, and in the summer of 2017 worked as an outpatient pharmacy intern with the Mayo Clinic Health System in Rochester, MN. Currently, Chris serves on SDSU s American Pharmacists Association-Academy of Student Pharmacists executive board as the Chapter Delegate. He also is currently on the leadership team of the Barnyard Cadets student group, a group which focuses on fostering campus tradition, and works as an Admissions Ambassador for SDSU where he interacts with prospective students and their families as they navigate the college search process.

Graduate of the SDSU College of Pharmacy & Allied Health Professions in Completed PGY1 residency at the University of Nebraska Medical Center in 2002 and a PGY2 residency at Barnes-Jewish Hospital (Critical Care) in Currently teaching responsibility focus in the areas of medical microbiology and infectious disease pharmacotherapeutics. Precepts infectious disease and internal medicine advanced pharmacy practice experiences (APPEs). Authored or co-authored 9 publications related to infectious diseases and/or antimicrobial stewardship, including a textbook chapter for infectious diseases in critically ill patients. Delivered 8 invited presentations related to infectious diseases and/or antimicrobial stewardship, including a presentation for a national audience at the American Society of Health System Pharmacists (ASHP) Midyear meeting in Las Vegas, NV (December 2016). Honored as the 2017 Pharmacist of the Year by the South Dakota Society of Health-System Pharmacists.

7 Tiffany Montalvo Laboratory Support Specialist College of Pharmacy Cell Phone: (605) Address: Tiffany Montalvo is the Laboratory Support Specialist for the Medical Laboratory Sciences Program at South Dakota State University. She also works with the Pharmaceutical Sciences department. She is a 2015 alumni of South Dakota State University, graduating with her B.S. in Medical Laboratory Science. Tiffany is responsible for overseeing all instrumentation within the labs as well as the inventory. Her sole responsibility is to help with lab preparation and assist the students within the lab. Tiffany also provides supplemental instruction to students during their resource hours, as she has extensive knowledge of the program and profession. Tiffany is also active within her profession by serving as a board member at large for ASCLS-SD. Through this she has maintained her education and supports upcoming new professionals within the medical laboratory science field.

8 April Nelsen Medical Laboratory Science Instructor/ Clinical Coordinator College of Pharmacy and Allied Health Professions Cell Phone: (605) Address: April Nelsen is the instructor for Clinical Chemistry I & II with labs, Diagnostic Immunology with Lab, Management and Education, and Advanced Medical Diagnostics classes for the Medical Laboratory Science Program at South Dakota State University. She is also the Clinical Coordinator for the Medical Laboratory Science Program. She is a 2015 alumni of South Dakota State University, graduating with her M.S. in Microbiology and her B.S. in Clinical Laboratory Science and Microbiology in April is also the MLS club advisor, the chair of the MLS Admission s committee, and a member of the MLS Academic Affairs committee. She serves as the Vice Chair for the American Society of Clinical Laboratory Science National Scientific Assembly of Immunology and Immunohematology and is the ASCLS Region V Treasurer/ Secretary.

9 Search Committee Member: Om Perumal Office Phone: (605) Cell Phone: (605) Department Assistant Assistant s Professor and Department Head/Graduate Program Coordinator Department of Pharmaceutical Sciences Omathanu.Perumal@sdstate.edu Emily Trias Emily.Trias@sdstate.edu Om Perumal serves as the Department Head for Pharmaceutical Sciences and is serving in his fifth year in this role. He has a BS degree in Pharmacy, an MS degree in Pharmaceutics, and a PhD in Pharmaceutics from India. He joined the faculty at SDSU in He had served as the Director of Translational Cancer Research Center at SDSU from As Department Head, he oversees the academic programs, research and outreach activities of the department. The department offers courses in pharmaceutical and biomedical sciences in the professional pharmacy program. In addition, the Department offers a PhD program in Pharmaceutical Sciences. The Department has an active research program in cancer and neurological diseases. Dr. Perumal maintains an active research program in drug delivery. Dr. Perumal s research group has developed several new drug delivery systems. The patented drug delivery technologies have been licensed to a start-up company. Dr. Perumal has published over 50 peer-reviewed papers, several book chapters, and made over 100 presentations in national and international meetings. He is the recipient of Patricia Walker Skin Cancer Research Award from Skin Cancer Foundation, F.O. Butler Award for excellence in research, and Intellectual Property & Commercialization award from South Dakota State University.

10 Search Committee Member: Shafiqur Rahman Professor of Pharmaceutical Sciences Office Phone: (605) Cell Phone: (605) College of Pharmacy and Allied Health Professions Dr. Shafiqur Rahman is a Professor of Pharmaceutical Sciences at South Dakota State University (SDSU). Dr. Rahman received Ph.D. in neuropharmacology from the Memorial University of Newfoundland School of Medicine in Canada and he has completed postdoctoral fellowship from the Department of Psychiatry at Indiana University School of Medicine in Indianapolis. He has held academic and scientific appointments at several Universities in the USA and Canada before joining faculty at SDSU in Dr. Rahman specializes in neuropharmacology/neuroscience research and pharmacology education. He has more than 20 years of experience in drug addiction and neuropsychiatric research and has served as PI/Co-PI for grants/contracts totaling over $1.3 million from a number agencies, including NIH/National Institute on Drug Abuse and National Institute on Alcohol Abuse and Alcoholism. His current research focus is drug discovery in neuropharmacology, i.e. the development of novel therapeutic candidates for the treatment of drug addiction, neuropsychiatric disorders and neuropathic pain. Dr. Rahman s research and scholarship has resulted in over 120 peer-reviewed publications, 150 abstracts/research presentations, and 3 books or 7 book chapters. He teaches pharmacology, neuropharmacology and drug addiction related courses to professional pharmacy (PharmD) and graduate (PhD) students at SDSU. He has mentored many undergraduate, graduate students, including five for PhD degrees from SDSU, and postdoctoral fellows. He has served as editor or editorial board member for a number of neuropharmacology and drug addiction or neuroscience related journals. Dr. Rahman has served as a reviewer for grants from several extramural funding agencies. He received numerous awards and recognitions for teaching, research and service including, the Award from the American Foundation for Pharmaceutical Education, Distinguished Researcher/Scholar Award for research and TRiO Students Choice Award for teaching from SDSU. He is a member of the American Association of Colleges of Pharmacy, American Society for Pharmacology and Experimental Therapeutics and Society for Neuroscience. In 2016, Dr. Rahman became an Academic Leadership Fellow of the American Association of Colleges of Pharmacy (AACP).

Schramm, PharmD, BCPS Director of Pharmacy Education & Academic Affairs Mayo Clinic Office Phone: (507) Cell Phone: (507) Dr. Schramm is the Director of Pharmacy Education and Academic Affairs Mayo Clinic; Assistant Professor of Pharmacy Mayo Clinic College of Medicine and Sciences; and PGY1 Pharmacy Residency Program Director Mayo Clinic Hospital in Rochester, MN. Garrett received a Doctor of Pharmacy degree from South Dakota State University College of Pharmacy and Allied Health Professions in 2004, with subsequent completion of a PGY1 and PGY2 Critical Care Pharmacy Residency at Barnes-Jewish Hospital in Saint Louis, MO. Prior to accepting an education administrative position in which he oversees education and training for more than 1,100 pharmacy staff and 12 postgraduate residency programs at all Mayo Clinic sites, Garrett worked as a Medical ICU pharmacist and clinical researcher. He has presented scholarly topics nationally and internationally. Garrett has served South Dakota State University as a member and current chair of the College of Pharmacy & Allied Health Professions Dean s Advisory Council.
