2 1 ST -6 TH GRADE AFTER SCHOOL PROGRAM NORTH WILDWOOD RESIDENTS ONLY BEGINS MONDAY 10/1/07 Boys and Girls in Grades 1st-6 th Mon-Fri from 2:15pm- 5:00pm (Early Dismissal Days 1-4:00pm) ** Registration is by Appointment Only. Parents/Legal Guardians must secure a copy of their child s immunization records and then make an appointment to come to the Recreation Center to fill out a registration packet before a child can attend the program. Appointment times will not be granted during normal After School Program Hours. Incomplete registration packets will not be accepted Registration Appointments Available beginning Monday 9/17 Call (609) to make an appointment Parents/Legal Guardians should bring the following information to Registration Appointment: 1. Child s Name, Age, Birthday, Grade, and School 2. Copy of Child s Immunization Record *if provided last year, then we should have a copy 3. All important Contact Phone Numbers and Names 4. A Reliable Emergency Contact *Name and Phone Number 5. All Contact Information for People you want to include on Authorized Pick-up/Visit Sheet *full name, full street address, city, state, zip, and phone number 6. Complete list of Medications for your child *if applicable 7. Child s Physician Info * Practice Name, Doctor Name, Doctor Phone 8. Child s Dentist Info *Practice Name, Dentist Name, Dentist Phone 9. Name of Family Hospitalization Plan 10. Copy of Custody Agreement * if applicable 11. Any and All Health Information regarding child *including any special needs information TOT-TIME BEGINS TUESDAY 10/2/07 Tuesdays and Fridays 10:00am - 12:00pm Playgroup at the N. Wildwood Rec Center for boys and girls who are not school aged Children are supervised by their parents as they learn social skills and how to interact with others No Registration or Fee Call for more information

Wildwood Rec Center (900 Central Ave) Monday and Wednesday Night Games in N. Wildwood, Sea Isle, and Middle Twp. Practice Schedule: Determined by Coach $50.00 per child Registration Fee, Completed Code of Conduct and Copy of Birth Certificate Required at Registration FALL ROLLER HOCKEY LEAGUE REGISTRATION BEGINS MONDAY - 9/8/08 Outdoor Roller Hockey League for Boys Ages years Old Registration Forms available online or at Rec Center Register at the N.
