1 SUNCOAST WALDORF SCHOOL Enrollment and Financial Agreement In consideration of Suncoast Waldorf Association, Inc., d/b/a/ Suncoast Waldorf School, reserving a place for the student named herein and making available its teaching services, facilities and resources, The undersigned hereby enroll(s) in the class, subject to final acceptance by the school following the submission of this agreement for the entire school year, and agree(s) to the following terms, conditions and obligations. 1. Enrollment is effective upon execution of this agreement by the school s authorized representative and is for the entire school year. All tuition and charges shall be due and payable in accordance with the terms of this agreement, without regard to absence, illness, suspension, withdrawal, or dismissal of such student or for any other reason, except as provided in section The undersigned agree(s) to provide the school with a completed school physical form (your physician has these) and an official (blue copy, not a photocopy) immunization form or official signed immunization waiver and any necessary student record forms required by the school, all of which shall be supplied prior to the opening day of the school year. 3. The undersigned understand(s) that retention of a student during any academic year and re-enrollment for any subsequent academic year remains conditioned and contingent upon fulfillment of the obligations as set forth herein, and upon satisfactory scholastic and behavioral performance by the student consistent with the published standards of Suncoast Waldorf School, a copy of which has been or shall be provided to the undersigned prior to the student s initial attendance at the school. Receipt of such standards shall be deemed to constitute full and complete knowledge of the contents. 4. Suncoast Waldorf School reserves the exclusive right to fairly and appropriately discipline the above-named student (up to and including dismissal) for neglect of school work, excessive absenteeism or tardiness, poor student citizenship, misconduct, or failure to follow the published rules and regulations of the school. The determination of the school, as expressed by the Board, as to all such matters will be conclusive, pursuant to the published procedures set forth in the Parent Handbook. Furthermore, the undersigned agree(s) to accept the established rules and policies of the school and those rules and policies from time to time promulgated by the Board of Trustees or the Faculty and Administration of the school. The undersigned understands that the school retains full rights to require withdrawal of the student for just and reasonable cause as is deemed adequate or necessary by the school, which cause shall be communicated to the undersigned in writing. The undersigned agree(s) to pay all debts then owed to the school and not seek any legal action against the school, its employees, staff, volunteers, agents, officers or Board of Trustees. 5. The school agrees to provide educational services appropriate to the grade level in which the child is enrolled, according to the Waldorf curriculum. The school is not equipped with special resources required to remediate serious and persistent learning challenges. The undersigned agree(s) to pay for such tutoring, external consultation or special services that the school does not provide and which the school justifiably determines the student may need. An appropriate representative of the school will consult with the undersigned prior to the imposition or recommendation of any such additional expense if it is more than incidental. 6. The undersigned hereby authorize(s) and give(s) full consent for the school to publish or copyright without charge or fee all photographs, videos or other depictions or recordings taken or obtained of the enrolled child while engaged in any and all school programs in furtherance of legitimate school objectives, to include marketing and advertising the school s services, programs and resources. 7a. Monthly Installment Tuition Payments: Suncoast Waldorf School, as a convenience to participants, permits monthly installment payments (as distinguished from a lump sum pre-payment). The monthly installment tuition payments begin June 15, 2011 and run through May 15, Such continued payments may be cancelled or refunded in the sole discretion of the school. In the absence of such cancellation or refund, however, any payments for the balance of the unpaid tuition will be due whether or not the above-named student is attending the school at the time the payments become due. 7b. Automatic Bank Account Debit / Automatic Credit Card Debit - Please complete the attached form, electing to either pay via an automatic bank account debit or an automatic credit card debit. Please return the completed form with your signed Enrollment and Financial Agreement. If you should choose to pay your monthly installment tuition payment by check, you will be charged a $25.00 administrative fee for each such payment. 7c. Monthly installment tuition payments are due on the 15 th of each month for the following month. A grace period extends to the 20 th day of each month, after which a $50.00 late fee will apply. The undersigned understand(s) that if tuition (including any late fee) is not paid by the 1 st day of the month the student will not be allowed to attend classes unless and until a meeting has been arranged with the Finance Director and payment ensured.

After sixty (60) calendar days of tuition being past due, unless other provisions are made (such as making weekly rather than monthly payments), the school will work through outside means (action through an attorney or the assistance of collection agencies, etc.) to collect the monies owed. The school also reserves the right to report the contract violation and past due status of the account to any/all credit agencies. At this point, too, the school reserves the right to withdraw the child(ren) from the school. 8. If a student is not in attendance for thirty (30) consecutive calendar days, and if no prior written understanding or arrangement is made with and approved by the school with regard to any such non-attendance, the Suncoast Waldorf School will consider the student withdrawn and enrollment will be terminated. Tuition obligations are still binding for the remainder of the school year, as otherwise stated within this contract. 9. The undersigned agree(s) that the selection of payments with the associated fees and charges, as indicated below, have been fully disclosed and explained, and the undersigned fully understands that obligation. The undersigned agree(s) to pay all attorney and/or other collection costs and fees incurred by the school in the collection of any amounts due. 10. When a child is withdrawn, the undersigned will give Suncoast Waldorf School a minimum of two weeks prior notice in writing and agrees to participate in an exit interview as well as complete the necessary withdrawal forms. 11. Supplies All supplies are the property of the school. If a student should leave the school for any reason, the student will be given his/her used workbooks and artwork. All other supplies will be retained by the school. 12. Community Support Our school is a developing school and as such we rely heavily on support from the families in our community. Each family is required to perform 40 hours of community support service OR make a monetary donation (in lieu of service time) to the school. The amount of the monetary donation will be determined during a meeting between a family member and the school s Administrator. Community support hours may include the following: WORKDAY SERVICE CLEANING FOR OPEN HOUSES MISCELLANEOUS SERVICE (i.e. office volunteer, committee work, etc.) Failure to complete the required community support or failure to make an appointment to discuss the monetary donation in lieu of service time will be factors taken under consideration during the re-enrollment process of your child(ren). If your family receives tuition assistance, you are required to serve 10 hours of community service each school month (September through June total 100 hours). If as of December 31, 2011 your family has not served a minimum of 40 hours, the school reserves the right to rescind your tuition assistance benefits for the balance of the school year. 13. Annual Giving It is suggested that each family participate in our ANNUAL GIVING campaign which supports the life of the school. A minimum contribution of $10.00 is required to qualify as a participant in the campaign. 14. It is expected that each family in the school will embrace the philosophy of Waldorf education and, towards that end, will do independent reading, attend class meetings, participate in workshops and strive to create a home life that is compatible with Suncoast Waldorf School s philosophy. 3 Page Three Tuition Protection Plan (Purchase not required; however, the form must be signed in order to enroll) Expenses to support the school and curriculum are budgeted and incurred on an annual basis. The school commits to these expenses, at the beginning of each school year, based on the parents enrollment commitment to the school. As a result, Suncoast Waldorf School offers the following. The purpose of the Tuition Protection Plan is as follows: To lessen the parent s burden of full, prepaid tuition payment if family circumstances change, requiring the child to withdraw during the school year (after May 31, 2011). To lessen the school s burden from a shortfall in revenue in the event a student does withdraw during the school year. The Tuition Protection Plan works as follows: The Plan is funded by an additional one-time fee of $ per student and is non-refundable. This is to be paid with your signed enrollment agreement. Please check one of the following: If tuition payment is made in full for entire year:! With the Tuition Protection Plan: Family is refunded 50% of any unused tuition.! Without the Tuition Protection Plan: Family is refunded 0%. If tuition payment is made in monthly installments:! With the Tuition Protection Plan: School is reimbursed 50% of any remaining, unused tuition.! Without the Tuition Protection Plan: School is reimbursed 100% of all remaining, unused tuition. Please Note: If a student s enrollment is withdrawn May 31, 2011, full tuition is not due. Suncoast Waldorf School will refund only the $ Tuition Protection Plan fee and no other fees and deposits. The Plan only applies to school year enrollment. Upon withdrawal, please make arrangements to meet with the Financial Director. The Plan does not apply for temporary absences due to illness or medical conditions. ***If more than one person signs this Enrollment Agreement, each person is fully and personally obligated to keep all promises made in this Agreement. Any person who is a guarantor, surety, or endorser of this Agreement is also similarly obligated. Any person, who assumes or is assigned these obligations, including the obligations of a guarantor, surety or endorser of this Agreement, is also obligated to keep all the promises, duties and obligations contained in this agreement. The School may enforce its rights under this Agreement against each person individually or against all the undersigned jointly. This means that any of the undersigned may be required to pay all of the amounts owed under this Agreement. As a convenience to the parents arrangements may be made with the School Director to pay the balance, if any, by the last day of the school year. After the last day of the school year, if arrangements have not been made with the School Director, a late fee of $50 will be charged in addition to the balance and payment of all such balances shall be a condition of re-enrollment for the subsequent school year.*** Parent/Guardian Signature (s) Accepted for Suncoast Waldorf School by: Print Name and Sign

4 Page Four Days Probationary Period (form required for enrollment) We wish to welcome every student into the school with the hope that our philosophy and methodology will meet the needs of each child. A probationary period of 90 days gives both the school and the family an opportunity to see if there is a good match. Final acceptance will be given to new students after this probationary period has passed. The probationary period does not recur, nor will it be extended, except for those students for whom the faculty deems it necessary or appropriate. ( ) Children are accepted conditionally: Some children with significant learning or behavioral challenges may be conditionally accepted in order for the school to determine if the Waldorf curriculum and this school with its available resources are suitable for them and whether with supplemental services and supports (parent/gaurdian s responsibility), the child might be successful. Where this paragraph is checked, the child has been determined to be in this category. In such case, it is recognized and agreed to by the parties that it may take additional time to determine whether the school is able to offer the child an appropriate and effective education. If the school determines at any time that the school is not able to effectively provide the child with an appropriate education, then the school may, at its discretion, withdraw the child from the school, regardless of the above probationary period. If, after careful consideration and work between the parents and the teacher, it is determined or recommended that the child be withdrawn from school, the school will maintain the following policy: If the School determines it is in the best interest of the child to withdraw from Suncoast Waldorf School: All unused tuition will be refunded. If the parents determine that it is in the best interest of the child to withdraw from Suncoast Waldorf School: All financial obligations will still need to be met, unless otherwise modified or adjusted by the school in special circumstances at the school s sole discretion. 5 Page Five Florida State Scholarships and School Tuition Assistance Programs 1. Florida Step Up For Students (Pride) Scholarship ($4,106 max. p.a. if eligible) Parent must apply and follow up. Apply online at beginning March 8, Early application is imperative as funds are limited. Parents must receive an award letter from Step Up For Students Scholarship in order to prove they have been approved for the funds. 2. Florida McKay Scholarship Parent must apply and follow up. Children who have received an IEP from a Florida Public School may be eligible. Parent must receive an eligibility letter to prove they have been approved for the funds. Visit for more information. Suncoast Waldorf Tuition Assistance The following documents required by April 22: a. Last two years tax returns b. Most recent paystub for all those earning in household c. Cover letter explaining why you want child to attend the school, and what you can commit to paying each month (for 12 months), including an explanation of any mitigating circumstances which have created the need for tuition assistance. The Finance Committee and Board of Trustees will determine the total amount of tuition assistance to be awarded during the school year and the guidelines for the maximum amount per request. Submission of a request is not a guarantee that you will receive any assistance.

7 Page Eight First Grade Readiness Assessment Program Suncoast Waldorf School will perform a first-grade readiness assessment for children who are in kindergarten in the latter portion of the school year. Your child s kindergarten teacher will advise you as to whether your child will be assessed and what is involved. In general, the assessment takes place over an eight-week period for a few hours twice a week. The children will play and be asked to do certain movements to see if they are developmentally ready to move to first grade the following September. The assessment will take place during the afternoon portion of the kindergarten day after lunch. All eligible children will be required to participate in the assessment in order to be considered for the following September s first grade placement. There is a small charge for the assessment, including additional supply fees. As always, if parents should have any questions regarding the program, your child s teacher will be able to help you.
