2 BIA APPLICATION PROCESS AND GRANT PROCEDURES Student should apply for admittance to an accredited college or university while applying for grant assistance. Students are to be directed to the Financial Aid Office of the college they plan to attend, or are attending, to have their financial need evaluated. A student should request that an official financial needs analysis be forwarded to the Bishop Paiute Scholarship Committee, by one of the college s Financial Aid Officers. This analysis will include all sources of financial assistance, which are to be applied toward meeting the student s total need. The Bishop Paiute Scholarship Committee will review the official analysis and base the amount to be awarded on available funding. ANY INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED APPLICATION DEADLINES: Documents need to be complete and must be received no later than the established deadline dates. Spring Semester Thursday, December 18, ) ELIGIBILITY REQUIREMENTS: BIA Grants are available to full-time students that are enrolled or accepted for enrollment, in an accredited college or university and demonstrate financial need. 2) UNIVERSITIES AND COLLEGES: The college or university a student elects to attend must be accredited by a nationally recognized accrediting agency, or be an institution whose credits are accepted on transfer by not fewer than three institutions that are accredited by a nationally recognized accrediting agency. 3) REQUIRED APPLICATION DOCUMENTS: Students must have a completed BIA grant application, by the deadline, to be considered for the semester. After all application documents are submitted, an acknowledgement letter will be mailed to the student notifying them if their application status. It is the student s responsibility to provide all of the documents needed to complete their application by the stated deadline. A complete application package must include: a.) A Complete BIA Grant Application. b.) A Complete FAFSA Application (Used To Determine the Student s Financial Need). c.) Current Tribal Enrollment Resolution/Verification. d.) High School Transcripts or GED, For First Time Students. e.) Official Financial Needs Analysis; Form provided and is to be completed by the financial aid office. f.) Current College Schedule and Final Grades from the Last Semester Completed, if applicable.

3 4) REASONS FOR DENIAL OF GRANT: a.) Application not submitted by the grant application deadline. b.) Incomplete application (missing information, transcripts, etc.) c.) Student not attending school, dropped out, or did not register. d.) Student is in default with school s Financial Aid Office or the Bishop Paiute Tribal Scholarship Program. 5) CONDITIONS FOR CONTINUATION OF GRANT: Students receiving grants must maintain acceptable academic standing at their college or university; maintain social conduct within the policies and rules of the institution attended, demonstrate satisfactory progress toward degree requirements and successfully complete the school year, or grant period, specified by the college. Grade reports of each student are to be reviewed at the end of each semester. Students earning less than a 2.0 GPA may be continued on a probationary basis, providing the college recommends their continuance. The recipient shall agree to the following conditions set forth by signing and dating this application form. a.) Student shall submit a new application each year. b.) Student shall submit their grades from their last semester attended. c.) Student shall be responsible for keeping the Bishop Paiute Scholarship Committee informed of his/her status once he/she has entered college. A change of address, change of income, intent to transfer to another institution, and academic progress shall be made known as soon as possible to the Student Services Coordinator. All changes must be made in writing for documentation. d.) A new Financial Needs Analysis must be submitted every year. 6) ACADEMIC PROBATION PROCEDURES: Students currently receiving funding will be put on probation IF one, or more, of the following events occur. a.) Student s G.P.A springs below the minimum 2.0 requirement. b.) Students who withdraw to less than full-time status (12 units), or under the full-time credit requirement of the institution attended. c.) Special circumstances may be considered but they must be submitted in writing to the Bishop Paiute Scholarship Committee. By signing this document and by my acceptance of any scholarship award/funding, I am acknowledging my responsibility to comply with the following terms. I understand that I must be enrolled as a full-time student (12 units or more) Official institution verification of Full-time status for the spring semester must accompany this application. Completion of less than full-time during the spring session will result in probation penalties and/or repayment of monies as stipulated by the Scholarship Committee I agree to immediately notify the Student Services Coordinator of any circumstances that cause any changes in my academic standing or status. Notification must be submitted in writing and within 10 days of those changes. I agree to submit my semester grades and class schedules as requested for each funding cycle. I agree to complete an information release form with the college or institution that I attend to enable the Student Services Coordinator and/or the Education Director to monitor/verify my attendance status and assist me if necessary. I agree that the Scholarship Committee has my consent to use my name for information related to the scholarship for various media use such as local, state, national, and internet recognition as deemed appropriate by the scholarship committee. I understand that I ll need to complete a Community Service Project. I understand if any penalty/repayment of scholarship monies are assessed upon me, that a repayment plan will be established by the Scholarship Committee and failure to adhere to the plan, may result in retention of 100% of per capita/tribal distribution monies. I will adhere to the terms in the signed Student Agreement It is necessary to reapply for the Bishop Paiute Tribe s BIA Grant Program each academic year or when transferring to a different school. I authorize the school to release grades, financial information and class schedules to the Bishop Paiute Scholarship Committee and its personnel.
