2 High School and Honor College Stipend Request Guidelines Please read the guidelines below to ensure that you meet the minimum requirements for stipend receipt: A). High School Stipend Request Guidelines: 1. Must be an active member of St. Paul Baptist Church. 2. Graduating High School students are eligible for a one-time stipend to apply for tuition, books, and/or board at an accredited university and/or college. 3. Must provide Official acceptance letter to an accredited college program or course of study at an U.S. institution. 4. Must Complete Stipend Request Form. 5. Late requests forms will not be accepted. Deadline for request is August 12, Stipends will be sent to qualifying students universities and/or colleges. B). Honor College Stipend Request Guidelines: 1. Must be a member of St. Paul Baptist Church. 2. Continuing College Students must maintain a minimum of a 3.0 GPA at an accredited university and/or college. 3. Must provide Official Transcript from an accredited college program or course of study at an U.S. institution. Transcript must include grades from 1 st and 2 nd semesters and be sealed and unopened in the college s envelop. 4. Must Complete Stipend Request Form. 5. Late requests forms will not be accepted. Deadline for request is August 12, Stipends will be sent to qualifying students universities and/or colleges. Note: Completed Stipend Requests will be reviewed for accuracy by the St. Paul Baptist Church Christian Disciple Scholarship Committee. Scholarship Application Checklist Fully completed Stipend Application (including signed certification of information). Documentation of official acceptance to an accredited college or course of study at a US institution (Graduating High School Student). Official Sealed, Unopened Transcript with a minimum 3.0 GPA (Continuing College Students). 2

3 St. Paul Baptist Church College Stipend Request Form Please complete the requested form and attach proof of Official acceptance letter to an accredited college program or course of study at an U.S. institution for Graduating High School Student. A Continuing College Student must submit an Official Transcript in a sealed unopened college envelop. The transcript must come from an accredited college program or course of study at an U.S. institution. Incomplete request forms or lack of proof will void the stipend request. Completed forms with attach proof should be mailed via US mail by the August 12, 2016 deadline to the following: St. Paul Baptist Church, 1401 Allen Street, Charlotte, North Carolina Attention: St. Paul Baptist Church Christian Disciple Scholarship Committee.
