2 F o r e w o r d There have been many amendments and introduction of new policies made in medical services of ONGC which may not be known to our employees/ beneficiaries, especially our retired veterans. Therefore, an effort is being made to compile the relevant circulars / office orders / guidelines through this Hand Book which may serve as a handy reference and help in settlement of claims. 3 Medical Scheme 1. ONGC provides the best medical facility to its employees viz., a) Regular, their dependent family members. b) Retired employee and his/her spouse for life, on joining the Scheme by making lump sum contribution as per Rules. c) Family members of deceased employee who died while in service. Norms to avail the medical facility: i) Regular employee : His/her dependent spouse, children and parents Son : Up to 30 years of age or till he gets job whichever is earlier Daughter : Till marriage or employment, whichever is earlier. (Up to three children to those employees joined prior and up to two children to those who joined after the above date) ii) Retired employee : Employee and spouse for life. iii) Those who lost their lives : Medical facility to dependent parents till while in service the period of normal retirement age of the deceased, spouse for life and to his/ her children as mentioned in (i) above iv) Permanently disabled : Medical facility for life of these children. children of Regular / A Certificate stating that they are unable to Retired and of those who earn their living be produced to Medical lost their lives while Section, Chennai. in service/on duty/vrs How to avail medical facility i) Regular employee : Should produce Medical Photo Identity Card issued by ONGC to ONGC Hospitals/ empanelled hospitals for availing indoor/ Outpatient treatment. The card must be surrendered to ONGC on retirement/transfer/vrs/resignation/ dismissal. ii) Retired employee : ONGC issues Medical Card to them which has to be produced to the empanelled Hospitals / Clinics for availing indoor/outdoor treatment iii) Employees keeping families : Medical token cards are issued by respective at a place other than HR Section to enable the employee s dependents place of posting avail medical treatment. This card is valid for one year.

4 Periodical Medical Check-up For early detection of occupational diseases, regular employees are sent for periodical medical check up. Other medical facilities namely, spectacles, artificial appliances, hearing aid etc are explained in detail in following pages. Credit facilities Procedure for Indoor Treatment An advice indicating the nature of treatment / disease from concerned Specialist of the Hospital for admission should be obtained for approval of the Head of Medical Services. Subsequently, Medical Section will issue Credit Authorization letter based on the approval of Head of Medical Services. In case of emergency and on holidays, employees and their dependents / retired employees and their spouse can get themselves admitted to empanelled hospitals on production of Medical Photo Identity Card. The inpatient treatment thus availed, will have to be regularized with the aforesaid formalities, on the next working day. The employee / patient / dependent should sign on the original bill of the hospital at the time of discharge.. Procedure for Outdoor Treatment For Retired Employees Employees on reference from outside Region are allowed Out Patient Credit (OPD) facilities. OPD credit facility is also allowed for costly investigations advised by Specialist of a panel hospital for Chennai based employees / medical token card holders and Retired employees. Declaration PAN Card TTA Change of Residence Reimbursement of med. Expenses. (Outpatient treatment) Reimbursement of med. expenses (Inpatient treatment) Non-working certificate be submitted every year to I/C HR-ER duly witnessed by either ONGC Executive or by any Executive of nationalized bank/psu. Submission of a copy of PAN card to Medical Section as per instructions of Ministry of Finance Eligible for medical facility at a place for which TTA was drawn. Any change in station for availing this facility shall be allowed after completion of 3 years. Medical Section should be intimated with the change of residence Limited to 7 months emoluments of pay in 12 consecutive months. Beyond this, approval of concerned Director is required. As per actuals subjected to admissibility.

5 CONTRIBUTION BY RETIRED EMPLOYEES FOR JOINING THE SCHEME (O.O.No.ONGC/ER/CP/MED/009 dtd ) Retired Executive employees are required to pay a lump-sum one time contribution by Cheque in favour of Finance & Accounts Officer, ONGC, Chennai for availing post-retirement medical facility, as per the level and amount mentioned against each. For in-patient/out-patient treatment in non-paneled hospitals, General treatment. reimbursement will be at par with rates of KEM Hospital, Pune. Dental treatment. Treatment cycle For dental treatment availed in non-paneled clinics, hospitals, reimbursement will be as per the rates of Ganga Ram Hospital, New Delhi. Normally the treatment cycle for a single disease is 21days. HUMAN INSULIN Patients using Human Insulin (analogue) in cartridge form, will have to bear the cost of PEN and NEEDLES from their own sources for utilizing the Inj. 7 Hearing Aid (O.O.No: ONGC/ER/CP/ MED/ 011 dt & NO.OHSS/1/1/2010 DTD ) Employees / retired employees and their dependents will be eligible (only twice after retirement) for reimbursement of cost of hearing aid in the prescribed format for : Type of Hearing Aid Conventional and bilateral hearing aid Digital hearing aid Recommended by Incharge, Medical Services at the Work Centre or empanelled ENT specialist supported by audiometric evidences. Two ENT Specialists, supported by audiometric evidences & three nos. quotations and final recommendations by CMS, DDN. Limit Rs.10,000/- (for one side) Life : 5 years Rs.30,000/-( for one side) Life : 5 years. Second hearing aid will be allowed only after condemnation certificate issued by ENT specialist Spectacles : (O.O.No: ONGC/ER/CP/MED/011 dated ) All employees with at least one year of service rendered in ONGC shall be eligible for reimbursement of spectacles/contact lenses. Besides, their spouse and dependent children covered under definition of Family may also avail the reimbursement within the ceiling. Reimbursement is as per actual or the maximum ceiling as given below : Level Ceiling W & A level Rs.3,000 S level & E-0 to E-4 Rs.5,000 E-5 to E-8 Rs.6,000 E-9 & above Rs.8,000 Reimbursement is admissible in a block of two years. i.e. Holiday Home block year. For regular employees reimbursement only on recommendation for spectacles by an eye specialist, production of cash memo, with specification for glasses and an entry in the service record of the employee. Spectacles For Retired Employees: (O.O.No: ONGC/ER/CP/MED/001 dated ) Eligible for retired employees, their spouse, dependent family members of deceased employees (who s death while in service) excluding parents. Reimbursement claim in prescribed format be sent to HR-ER/Senior Citizen Cell. 8 In case of visually challenged employees who require usage of dark glasses goggles and their dependent children, the cost of dark glasses goggles/ spectacles/contact lenses may be reimbursed as per actual within the following limits: Level Ceilings Visually challenged for dark glasses goggles Spouse & dependent children for spectacles/contact lenses W&A Level Rs.750 Rs.1500 S level & E-0 to E-4 Rs.1250 Rs.2500 E5-E8 Rs.1500 Rs.3000 E-9 & above Rs.2000 Rs.4000 Reimbursement of cost of IOL (O.O.No: ONGC/ER/CP/MED/011 dated Normal IOL / Multifocal IOL cost ceiling is Rs.2800/- Persons undergoing cataract surgery requiring IOL implantation will be eligible for reimbursement Cost Ceiling : Rs.7500/- for Monofocal foldable IOL subject to: Foldable Acrylic Monofocal IOL (Hydrophobic/hydrophilic) of brand Bausch & Lomb, Carl-Zeiss, Rayner, Alcon & Abbot Medical Optics (AMO) or equivalent make. Operating surgeon/beneficiary to attach IOL sticker with batch number bearing the signature and stamp of operating surgeon on it) Cost Ceiling : Rs.7500/- for monofocal foldable IOL & to be claimed in the prescribed format. Lasik Surgery (O.O.No: ONGC/ER/CP/MED/011 dt ) Eligibility Condition Ceiling Claim Employees and their dependents up to 45 years of age Eligible for persons using spectacles or contact lenses of +/- 6.0 diopter and above. Rs.15,000/- for one eye including medicines and other expenses In prescribed format with prescription of eye specialist and a copy of test report Medicines On Credit to Chennai beneficiaries from Apollo Pharmacies : There are more than 100 outlets of Apollo Pharmacies across Chennai City, which supply medicine on Credit to ONGC employees (Chennai beneficiaries only). To avail this facility, one has to bring the following to ONGC Medical Section, Chennai. i) Original prescription ii) One Xerox copy of prescription where-in name, designation, CPF number, relationship to employee and location shall be mentioned. Then, the ONGC Doctors (Regular) shall sign on the original prescription (for record) and also on the Xerox copy mentioning the number and quantity of medicines to be supplied. The original prescription shall be retained by the employee/beneficiary and the original signed Xerox copy of the prescription to be handed over to the Apollo Pharmacy against which medicines shall be supplied on credit. Note: In case of remote locations (especially for retired employees) our beneficiaries may approach the nearest Apollo Pharmacy for speedy supply in which case, the formalities shall be carried out by the Apollo Pharmacy personnel.

9 IVF (O.O.No: 16(68)/97-Welfare dated ) Regular employees are allowed for IVF treatment with the monetary ceiling of Rs.50,000/- (including cost of medicine per cycle) with a maximum limit of 3 cycles. (Not revised). SMALL FAMILY NORMS (O.O.No: 11(3)/96-Welfare dated ) Category Employee or Spouse to undergo Family Planning operation. In case of male employee, age should not be above 50 years and his wife should be between 20 to 45 years. In the case of female employee, she must not be above 45 years and her husband must not be over 50 years of age. The employee should not have more than two living children. Condition Sterilization certificate must be issued by a hospital. The employee will be entitled to one special increment and a cash incentive of Rs.300/- Consultation charges for out-patient treatment wef Sl. Category Consultation charges No. 1.
