1 Fall 2011 Dear Perspective EMT Course Student: This letter is to clarify Montana State University and Extended University, Office of Continuing Education s role in the EMT Basic Course offering. John Alston, the EMT Basic Course instructor, has contracted with Extended University, Office of Continuing Education to provide logistical support services for this offering. In addition to providing logistical support services, we award academic credit to those students who are qualified and pay the additional fee. The Office of Continuing Education is not involved in the course content, and we have no responsibility related to testing. Thank you for your interest in the EMT Basic Course. 2 Dear Prospective EMT Student: Thank you for your interest in the EMT Basic Course being offered through Extended University, Office of Continuing Education at MSU. I am John Alston the course instructor. I have been an EMT in the Gallatin County for twenty years and have taught hundreds of EMT and First Responder students in that time. Please read the EMT Basic Course application carefully and completely before submitting to MSU. There are strict guidelines and requirements for this course. Prerequisites and Ongoing Requirements: State of Montana Requirements of EMT Basic Students: * At least 18 years of age * High School Graduate or GED equivalent * No prior felony convictions * Personally approved by instructor and medical director * Attain CPR License for the Professional Rescuer prior to course completion date* * Work/volunteer for an agency that provides EMS services within 2 years of receiving license (local fire department, ski patrol, etc.) * Complete 48-hours of continuing education every 2 years * Complete 24-hour EMT Basic Refresher course every 2 years *CPR licenses can be attained through the Red Cross or Bozeman Deaconess Hospital for approximately $105. Bozeman Deaconess Hospital requires that students have a recent PPD (tuberculosis test) to cover time spent in class. PPD tests are available for a minimal cost from student health services, city or county health departments, Bozeman Deaconess Occupational Health, or a local physician's office. The cost range for PPD tests is $12.00 to $ Class and Study Commitment: It is estimated that you will spend hours in the classroom and 10 hours in Emergency Room observation. To say the least, this course is INTENSE!! It requires MATURITY, DEDICATION, and plenty of extra STUDY time. For every classroom hour you must match that with an hour of outside study. If you are an MSU student with a heavy class load, I would NOT recommend taking the course. In the past, MSU students who carried heavy academic class loads had a very hard time completing this course. Attendance is required. You may not miss more than two excused classes and must pass all exams with a minimum score of 70%. If you fail two tests, including make up tests, you will be dropped from this course. Non-credit and Academic Credit Options: You have the choice of taking this course as a non-credit or academic credit course. The tuition fee for NON-CREDIT is $ The course (MEDS 280) is pending approval for 6 academic credits. If approved the additional cost for the academic credits would be in the range of $1, (dependent upon the exact increase in fall tuition.) This is a self-support course and you will be required to pay this tuition fee even if you have hit the tuition cap. No staff/faculty fee waivers will be accepted for this course.

3 Testing: After successful completion of the course, and a personal recommendation from the both the medical director and myself, you will be required to pass a state exam to receive EMT certification. Testing for EMT certification is YOUR responsibility. At the completion of the course, we will make every attempt to provide a practical testing opportunity here in Bozeman for a fee of $ per student (amount is NOT included in the course tuition.) There are additional fees to take the NREMT written test ($70.00) and to obtain your state EMT license ($40.00). You are not obligated to test with us. If we are unable to provide testing in Bozeman, it will be necessary for you to find another testing location. Please note the cost may vary from test site to test site. Testing is usually offered one month after completion of the course, and it consists of five practical stations. Deposit and Final Payment (both non-refundable): If you are accepted to the course, we must receive a non-refundable deposit of $150 IMMEDIATELY after acceptance (following interview). Payment can be made by cash, personal check, MasterCard or Visa. Final course payment (also non-refundable) is due by Friday, August 12, Personal Liability Insurance: Personal liability insurance is required of all students by the Bozeman Deaconess Hospital. Please be prepared to either show proof of insurance, or to make a payment of $27 to cover one semester of liability insurance at the first class. They will be held on Saturday, April 30, Class Schedule: Tuesday and Thursday Evenings From: August 30, 2011 December 8, 2011 Time: 6:30 p.m. to 10:00 p.m. There is also one required Saturday session to be scheduled. This is the best EMT course offered in the state and when you graduate you'll know that you have earned it!
