1 Custom Fit Students, During the past year, we have been working towards making our enrollment process more lean and efficient for our small team. We will be modifying our schedule and course change procedures for students registering for OWATC programs. The Enrollment Office located in the Student Services Building, will now be handling all schedule and course changes. With that, all Custom Fit students will now be required to adhere to the policy and procedures set by the Ogden Weber Tech College. These changes will take effect as of April 1st, *Your individual companies have also been contacted by their Custom Fit Representative and they are aware of the upcoming changes. What this means to you: Students will be required to do schedule and course changes in person, at the Enrollment office, located in the Student Services Building. Phone calls, s, and texts will no longer be accepted. Students will no longer be able to take one or two days out of their schedule for personal reasons. Students may take a leave of absence (LOA) without completely withdrawing from the tech college. During a leave of absence, students are not charged tuition. A leave of absence is good for two, five school calendar day periods or one ten school calendar day period per fiscal year (July 1 to June 30). To take a LOA, students must submit a leave of absence form to the Enrollment Office or through the website at least one day prior to the requested days off. A Leave of Absence can be used for vacations, sickness or other personal reasons. Applicable fees and rules apply for leave of absence. There are no excused absences. If you are called into work, your company will need to be aware that an absence will occur by not attending enrolled hours. When you go into enrollment to change your schedule, only students with rotating shifts will be given free work related schedule changes if the documentation from their company stating the employee/student participates in a rotating shift has been recorded. (If you are on a rotating shift, your company should have documented your name on a template form that was sent to them, returned to us, and then Custom Fit will have sent the information to Main Enrollment where the documentation is recorded) All other schedule changes are subject to the schedule change fee. Our main campus enrollment has convenient hours Monday Thursday 8:00am 6:30pm and Friday s 8:00am 4:00pm. Students should arrive no later than 30 minutes prior to closing. I just want to say it has been a pleasure getting to know each of you. I have enjoyed hearing about your successes and watching as you progress through the programs. I understand that this change will be a transition and will require some planning. Please be patient as we roll these changes into effect. Be proud to be a student and keep your focus on your goal. 2 of 2 POLICY 1. PURPOSE AND SCOPE The purpose of the policy is to inform students of the College s admissions requirements. 2. POLICY It is the policy of the College to operate under an open-door policy and serve all qualified individuals on an equal opportunity basis. The College is unable to accept international students at this time. 3. DEFINITIONS 3.1 High School Student: A student who is either currently enrolled or who is eligible to enroll in grades 9-12 in a public school or who meets the definition of a disabled student per Utah Code Annotated, 53A Transfer Credit: Credit from another institution reviewed and approved by an instructor as meeting program requirements. 3.3 Articulated Credit: Credit from another institution accepted per articulation agreements between the College and the institution. 3.4 Qualified Individual: An individual who meets program, course, and College policy requirements. 3.5 Open Exit: A course/program for which monthly tuition and fees are charged in advance. Schedule is determined by the student and a calculated end date is based on the start date and schedule. Tuition is charged for every enrolled hour based on the student s exact schedule. 3.6 Lockstep: A course for which a fixed cost is charged up front and the schedule has defined start, stop, and meet dates. 3.7 Course Based: A course for which a fixed cost is charged up front and the schedule is determined by the student. The calculated end date is based on the

4 530.7-PR 1 of 5 1. The College reserves the right to deny admission to a student enrolling or re-enrolling. 2. Adult Students 2.1 Potential students shall contact the Student Services Department to determine enrollment requirements for the program they are interested in. 2.2 Assessment services including testing, work samples, career guidance, and counseling may be used to determine a potential student s employability, abilities, needs, and interests, as deemed necessary. A photo ID is required for assessment. 2.3 Most programs require students to pay tuition and fees on a monthly basis. Several programs require students to pay by the term or course before the first day of class. 2.4 Students who are seeking a program certificate must be scheduled a minimum of 12 hours per week if attending day classes and 6 hours per week if attending evening classes. Exceptions may be made on a case-by-case basis by College administration. Students should contact their counselors for details. 3. High School Students 3.1 Students who are 16 years old or have completed sophomore year may enroll at the College and attend day or evening courses during the school year and/or during the summer. Students who have completed the eighth grade may enroll in classes specifically for high school students. Exceptions can be made on a caseby-case basis or for specially-designed classes. 3.2 Students interested in enrolling at the College should contact their career and technical education coordinator to ensure they meet enrollment criteria If students high school grade point averages are less than 3.0, they shall be required to test prior to entering the College. Students can make an appointment for assessment by calling the College s Student Services Department.

5 530.7-PR 2 of If students high school grade point averages are above 3.0, they shall be eligible for enrollment upon referral by their career and technical education coordinator. Individual programs may require testing prior to enrollment, regardless of GPA. 3.3 The career and technical education coordinator shall provide the following information to the College registrar: Transcript An unofficial transcript is sufficient for admission if a student is not seeking transfer/articulated credit If credit is requested, the coordinator shall provide the college with an official transcript in one of two ways: 1) mailed or delivered in a sealed envelope; or 2) ed as a PDF attachment or faxed. If provided through or fax, it must be clear that the document is directly from the school College Enrollment Authorization Form 3.4 High school students may choose to enroll at the College outside of the regular school day or year. Authorization from the high school is not required. 3.5 Students are required to pay an annual high school fee at the time of enrollment for most programs. 4. Home School Students 4.1 Home school students who are 16 years old or have completed sophomore-level coursework may enroll at the College and attend day or evening courses during the school year and/or during the summer. Exceptions can be made on a case-bycase basis or for specially-designed classes. 4.2 Students without documentation of a 3.0 GPA will be required to test prior to entering the College. Re-Enrollment 5.1 Re-enrollment or re-admission is dependent upon meeting the requirements for a qualified student. 5.2 Students who withdraw and then re-enroll within six months from their withdrawal date are assessed a non-refundable re-enrollment fee. 5.3 Students in a course-based program will, upon completion of a course, be assessed a non-refundable re-enrollment fee when: the student takes longer than five school days to enroll in the next course, or the student takes longer than ten school days from end date of current course to start the next course. 5.4 Students who withdraw and then re-enroll after six months from their withdrawal date must pay a non-refundable enrollment fee. 5.5 Students who have been out of school for six months or more shall be required to upgrade their assigned certificate to the current version. This may make some of the students completed classes obsolete. Exceptions maybe considered on a caseby-case basis by College administration. 5.6 Students with an outstanding balance shall be allowed to re-enroll at the College when all past-due tuition and fees are paid in full or arrangements have been made through the College Fiscal Services Office. 5.7 Students who have been suspended may be charged a re-enrollment fee.

Admissions Appeal 6.1 If admission or re-admission is denied, the applicant shall be notified within five school days. 6.2 In accordance with due process, if an applicant chooses to appeal an admission or re-admission denial, he or she needs to contact the director of student services or designee within five school days of receiving the admissions denial. 6.3 The appeal shall proceed as directed in the Informal Procedure for Addressing Student Complaints (policy #520.8). 7. Social Security Section 6109 of the Internal Revenue Code requires a student to provide his/her valid social security number to the College upon enrollment. The information is used to complete various federal reports required by the government and shall be kept strictly confidential. Students who refuse to provide this information are required to complete a waiver form. 8. Tuition and Fees 8.1 Monthly tuition is based on the number of hours students are enrolled. A fulltime student s course load is 24 hours or more per week. 8.2 Adult students enrolled in open-exit schedules are required to pay the current month s tuition and fees before beginning training. Thereafter, tuition and fees are due every month in advance of course work. Failure to pay in advance shall result in a student s schedule being suspended, a late fee assessed, and student initiated reactivation of schedule. Students who are not on the class roll shall not be allowed to access the instructional area/resources (does not include orientation or advisement). 8.3 Programs that are lockstep or course based require students to pay full tuition and fees in advance of the start date. 8.4 High school students are not charged for tuition. However, they may be assessed other fees.

8 530.7-PR 5 of Tuition does not cover the cost of books, supplies, and fees. 8.6 Individuals who are 62 years of age and above may enroll at the College in classes for which they may be qualified, on the basis of surplus space in regularly scheduled classes. Individuals are exempt from tuition only; individuals shall pay all materials and fees according to the College fee structure. 9. Compliance with Legal Obligations/Restrictions 9.1 Enrolling students are required to comply with any legal obligations or restrictions placed on them by the courts or other legal entities, e.g., protective orders, terms of probation or parole, and may not enroll in the program if such action would conflict with said restrictions. 9.2 Students who enroll and are found to be out of compliance with any legal obligations or restrictions as it would pertain to the program will be subject to suspension and/or withdrawal pending an administrative review.

PURPOSE AND SCOPE The purpose of the policy is to provide students enrolled in an open-exit schedule with a procedure which will allow them to leave the College for a short period of time without withdrawing. This policy does not apply to students with a defined-length schedule. 2. POLICY It is the policy of the College to provide a leave of absence (LOA), upon reasonable request, to students for a minimum of 5 or a maximum of 10 school calendar days per fiscal year (July 1 to June 30) in 5 school-day increments. Unused days cannot be carried over to the next fiscal year.

To take a leave of absence, students must be enrolled in an open-exit schedule and complete all of the following (program exceptions may apply): 1.1 Obtain a Leave of Absence form from the Enrollment Office. 1.2 Receive approval from their sponsor (if applicable). 1.3 Notify their instructors. 1.4 Return the completed Leave of Absence form to the Enrollment Office or submit it online. 1.5 Pay appropriate fee. 2. The leave of absence goes into effect the next day. 3. Students are responsible for all financial obligations which are incurred when taking a leave of absence. 4. Students must provide a return date at the time they request a leave of absence. 5. Students failing to return to class when their leave of absence has expired shall be responsible for any incurred charges. 6. Students may request a leave of absence in an emergency (e.g., illness, transportation problem, work conflict) by speaking to an enrollment advisor in the Enrollment Office. Enrollment Office personnel shall complete the appropriate forms on behalf of the student. Students may also complete the Leave of Absence form online.
