Application Qualifications Admission requirements (Main requirements. The number in parentheses corresponds to the number in the original document. Applicants who do not fall within any of these categories should contact the administrative office of the graduate school, the Daigakuin Kakari) (1) A person who has graduated or is expected to graduate from university by 31 March (2) A person who has completed or is expected to complete sixteen years of regular curricular studies abroad by 31 March (3) A person who has completed or is expected to complete sixteen years of regular curricular studies in a distancelearning program of an overseas educational institution offered in Japan by 31 March (4) A person who has completed or is expected to complete a course at an overseas higher educational institution while resident in Japan (limited to a person who has completed a 16 year course of education in overseas schools), which is assessed to be an educational facility within that country s education system, and has been specifically designated by the Minister of Education, Culture, Sports, Science and Technology by 31 March (5) A person who has completed on or after the date specified by the Minister of Education, Culture, Sports, Science and Technology or is expected to complete by 31 March 2013 a course in specialist education at a vocational school (limited to a person with a course of a minimum of four years in length and that otherwise meets the standards specified by the Minister of Education, Culture, Sports, Science and Technology) that has been specifically designated by the Minister of Education, Culture, Sports, Science and Technology. (6) A person so designated by the Minister of Education, Culture, Sports, Science and Technology in accordance with the Announcement No. 5 of 1953 from the Ministry of Education. (Note 1) 1

2 (7) A person who has been awarded or is expected to be awarded by 31 March 2013 a Bachelor s degree in accordance with the provisions of the School Education Act, Article No. 104, paragraph 4. (Note 2) (8) A person who has completed fifteen years of regular curricular studies abroad or who has completed fifteen years in a distance-learning program of an overseas educational institution offered in Japan, and is recognized by our graduate school as having completed specific units with high grades. (note 3) (9) A person who has completed or who is expected to complete a course at an overseas higher educational institution while resident in Japan (limited to a person who has completed a 15 year course of education in overseas schools), which is assessed to be an educational facility within that country s education system, and has been specifically designated by the Minister of Education, Culture, Sports, Science and Technology and who has been deemed by this Graduate School to have gained the necessary credits through excellent academic performance. (Note 3) (10)A person recognized through individual examination as having qualifications equivalent to or more than equivalent to those of a graduate of a university, and is aged 22 years or more on 31 March (Note 4 1), 2), 3)) (Note 1) Qualification (6) above includes a person who graduated from universities under the former University Ordinance and a person who graduated from academies under the organizational or incorporation acts of the various ministries. (Note 2) Qualification (7) above covers a person who has been awarded or is expected to be awarded a Bachelor s degree from a degree-granting institution or an accreditation and degree-granting institution. (Note 3) A person applying under qualification (8) or (9) must contact the administrative office (Daigakuin Kakari, indicated in section 6.(3)) before submitting an application. (Note 4) 1) Qualification (10) above refers to a person who does not fall under any of the qualifications (1) to (9) above but who has graduated from or completed their education at an educational facility equivalent to a four-year university course and who has been deemed by this Graduate School on the basis of the separate evaluation to possess academic ability equal to or greater than university graduates. 2) A person planning to apply under qualification (10) will undergo a separate evaluation before submitting an application and must contact the administrative office (Daigakuin Kakari, indicated in section 6.(3)) for instructions and submit the required documents by Friday 1 June ) A person determined through the separate evaluation to possess academic ability equal to or greater than university graduates will be permitted to apply for and sit the entrance examination. 2. You may answer either in English or Japanese. For Human Genetics and Biomedical Chemistry, the questions will be focused more on molecular biology and biochemistry. (2) An oral exam will be given to those who pass the written exam. (3) Depending on the results of the examinations, the number of students admitted may not reach the expected capacity. (4) *International Health/ Global Health Sciences is conducted entirely in English. (5) **International Epidemiology, Tropical Diseases, and Environmental Health Sciences are Associated Departments of International Health/ Global Health Sciences. (6)** Note that although all the exam questions will be given in English, lectures in Tropical Diseases and Environmental Health Sciences are conducted partly in Japanese. 4. (2) Those who have passed the written exam will be announced on the board in front of our main building (Igakubu Honkan) at the foresaid times and dates. 5. Announcement of the Result, Admission Procedure (1) Applicants who are accepted will be announced on the board in front of our main building (Igakubu Honkan) at noon, Thursday 20 September (2) Those who have passed the exam will receive a notice of admission by post by the end of February

4 (3) After receiving a notice of admission, he/she must complete the admission procedure from Wednesday 6 March 2013 through to Friday 8 March 2013 in accordance with the instructions sent together with the notice of admission. This includes the payment of the admission fee and the filing in of all required documents. If this procedure is not completed within the given time period, he/she will be regarded as not having entered. (4) Fees (tentative for Academic year Students who receive a Monbukagakusho scholarship are exempted from payment.) Your application and required documents must be enclosed in our specific envelope and sent by registered mail. If you do not have any Japanese stamps, you will need to include the equivalent amount in International Reply Coupons (IRCs; a single IRC costs about 130 in Japan) available at post offices worldwide. C Exam fee of 30,000 yen (by bank transfer). All applicants (The following students are not required to pay: International students receiving Monbukagakusyo scholarships.) Bank transfer Fill in the required fields on the specified form and pay at your nearest financial institution (not at a post office). (Do not make ATM or internet money transfers.) Payments should be made at a local bank branch (not at a post office), using the given form (do not use an ATM or Internet banking). The bank will give you the Furikomikin Uketorisho (i.e. receipt, form B) and Furikomikin Uketsuke Shomeisho (i.e. verification of remittance, form C). Certificate of graduation All Applicants Not required from those who finished or are expected to finish undergraduate study in medicine at the University of Tokyo. All applicants who have already graduated at the time of applications [not required for graduates of the University of Tokyo s Faculty of Medicine] Any student who is not a research student or a fulltime student in our Faculty or Graduate School must submit an official certification of their status as a recipient of a Japanese Government Scholarship. Including transcripts of pre-medical programs. Copies may not be submitted. Also, if you transferred to another university, please submit the transcript of academic records from your previous university as well. In the case of students graduating from foreign universities, the degree acquired must be indicated. Photocopies are not acceptable. Submit a certificate of the degree conferred, created by the National Institution for Academic Degrees or the National Institution for Academic Degrees and University Evaluation. F Research Plan Required only from those who are applying while keeping their position at government offices, schools, hospitals, and companies. 5 Describe the plan of your research at our graduate school in 2 pages of A4 paper. Physically handicapped applicants who may require special consideration for sitting the exams or attending classes should make a request for this to the administrative office indicated in section 6.(3), at the time of application. 7. Other important points (1) We will send you the exam card and notes on the exam using the return envelope you send us. If this still has not

6 reached you 3 days before the exam, contact our office (Daigakuin Kakari, indicated in section 6.(3)) and ask for instruction. (2) If you take a written exam other than for the field specified by the division you apply for, then the exam you have taken will be invalid. (3) Changing the content of the document submitted, or a refund of the exam fee will not be allowed for any reason after the application is submitted. (4) You are allowed to attach comments on your professional achievement written by the head of your institution (any format is acceptable) or copies of your publications such as papers, reports, etc. or both. (5) If you are keeping your position at government offices, schools, hospitals, and companies, when you follow the admission procedure (refer to paragraph 5 item 3), submit a written agreement (free form) by the head of your institution to ensure that you are able to concentrate on the study while at our graduate school. (6) International students must have an appropriate visa for admission to our graduate school when following the admission procedure. (7) The admission fee will not be refunded for any reason after admission. (8) Handicapped applicants should report to the administrative office indicated in section 6.(3), because special consideration or support might be necessary when the examination is administered or, after enrollment as a student. (9) Names, addresses, and other personal information obtained through the application procedure will be used for the: [1] selection of students (the processing of applications, selection); [2] announcement of results, and [3] completing of admission procedures. Furthermore, for successful applicants only, personal information will also be used for: [1] academic-related matters (registration, study, etc); [2] student support matters (health management, job-hunting support, fee waivers, scholarship applications, library use, etc); [3] tuition fee collection procedures. (10) The University way retroactively cancel admission of any student who has falsified personal information or academic record in his or her application form. (11) The examination results used in the selection of entrants may be used in future studies for the improvement of both the entrance examination and the education at the University of Tokyo. May
