1 Healthy Chicago Community Seed Grants May 5, 2016 The Public Health Institute of Metropolitan Chicago (PHIMC) in collaboration with the Chicago Department of Public Health (CDPH) is releasing a call for applications from Chicago-based community organizations to seed implementation of Healthy Chicago 2.0 strategies. Healthy Chicago 2.0 details a broad spectrum of strategies for ten priority areas, identified through our community health assessment process, and will guide CDPH and its partners as we address health inequities facing specific neighborhoods and communities. PHIMC will award up to six seed grants ranging from $10,000-$20,000 to launch and or expand community capacity to implement one or more Healthy Chicago 2.0 strategies. Applications will be due on June 6, 2016 by 4pm. 5 Submission Requirements I. Key Point of Contact for this seed grant including name, title, address and phone number II. Submit a statement of interest summarizing the following that is no more than 3 pages: a. Healthy Chicago 2.0 strategy (s) that you intend to initiative and/or expand b. A statement of your agency s capacity to implement the strategy (s) beginning July 1, 2016 and continuing beyond December 31, 2016 when the funding ends. c. A statement describing your agency s commitment to support further implementation of Healthy Chicago 2.0 d. The target health condition, community area and/or population (see pages 2-4 of this announcement) III. Budget detailing the use of no more than $20,000 to implement the strategy(s) IV. List of Board of Directors All applications must be submitted to rfp@phimc.org with SEED Proposal in the subject line by June 10, 2016 at 4pm. No late submissions will be accepted.
