Register for the Genetic Counseling Admissions Match through the National Matching Services (NMS) at gcadmissions/. 2. Complete CSU Stanislaus Graduate Application for Admission form. (Online) Please select MS Genetics from section 10a (pull down menu) *Note: Supplemental Application will be accepted in place of the Statement of Purpose section of the Graduate Application for Admission form. 3. Complete the online Supplementary Application for Admission form. the completed form to MSGCStan@csustan.edu. (Include your full name in the subject line). Give your full legal name and avoid abbreviations. You may use additional pages if you need more space to respond, but put your name on each page, and attach the pages to your application. 4. Pay the $55 non-refundable application fee (associated with Step 2). 5. Obtain official transcripts from each college or university attended other than Stanislaus State.
