You will need the associate s six-digit Employee ID to register. Then, your guide will help you pick your ways to participate. STEP 2 Tell us more about you: Complete (or retake) your Compass (Required to earn rewards) Complete (or retake) the RedBrick Compass health assessment a short questionnaire about your health to find your strengths and see where you can improve. Don t worry if you aren t sure of some of your numbers. Your health assessment will automatically update after you get your health screening. STEP 5 Choose your Direction and healthy activities Based on the information you provide, RedBrick will offer you personalized recommendations for healthy activities, and your HealthMap will point you in the right Direction. STEP 3 STEP 6 Learn your health numbers: Get a health screening Participate in a health screening (or full physical) to get a better picture of your health and submit your results to RedBrick Health. Please note that it may take up to two weeks for your screening results to appear in your RedBrick account. Earn Rewards You (and your Capital One Medical Plan enrolled spouse or domestic partner) can earn rewards for participating with RedBrick. 4 5

4 STEP 1 Getting started It s important to make the most of your time right from the start. RedBrick can help you find the best activities for you, so you can keep moving in the right direction. Register To register your online account, visit MyRedBrick.com/BeWellBenefits or get started over the phone by calling WELL ( ). Stay connected There are multiple ways to access RedBrick Health, including online from your computer or remotely from a tablet or smartphone. And with an Android or iphone, you can use the RedBrick Health app, which puts your favorite RedBrick tools right in your pocket. You ll need to create a RedBrick username and password to access your account through your device s browser or the ios app. Visit the App Store or Google Play, and use this code to download the RedBrick Health app: bewellbenefits 6 7

5 STEP 2 STEP 3 Complete your Compass (Required to earn rewards) When you provide your health information by completing the Compass health assessment, you ll get recommendations developed just for you. Answer questions about your daily nutrition, exercise and sleep habits as well as how you feel, what you do to stay healthy and how you cope with stress. You ll see your results, and it will be easier to measure the improvements you ve made to your health over time. Answer questions online, on your mobile device or by phone (written release required) to get feedback and insights about your health. Don t worry if you aren t sure of some of your numbers. Your health assessment will automatically update after you get your health screening. Get a health screening Knowing your numbers like blood pressure, cholesterol, glucose and more will provide a more complete picture of your health. There are several ways to complete your screening and submit your numbers: Get a health screening on site at Capital One Visit your healthcare provider and ask him/her to submit the completed Health Screening Form Get a health screening or physical at one of the Be Well Health Centers Visit a RedBrick partner lab or retail clinic Log in to your RedBrick account at MyRedBrick.com/BeWellBenefits to print your forms or vouchers, or call WELL ( ) for help. Please note that it may take up to two weeks for your screening results to appear in your RedBrick account. Health screenings are no longer required to unlock your rewards. 8 9

6 STEP 4 Get a Next-Steps Consult After you complete your Compass health assessment and a health screening, schedule a 15-minute phone call with a RedBrick guide to: review your assessment and screening results gain additional insights about your current health receive personal, expert guidance on what to do next What s Next? STEP 5 Choose your Direction and healthy activities Start by reviewing your HealthMap. Your HealthMap is a personalized path to success based on your unique health numbers, habits and goals. Within your HealthMap, you ll receive recommendations on which Direction to take. Directions are health focus areas such as Healthy Back, Weigh Less and Find your Balance. You ll need to complete your Compass health assessment before you can schedule your Next-Steps Consult

7 Once you choose a Direction, you can select the healthy activities that work best for you. RedBrick Track Log your daily healthy habits Health coaching Work one-on-one with a health expert RedBrick Journeys Online programs your way, at your pace Challenges Fun and social competitions RedBrick Track Track is a good reminder to exercise and eat clean. It s about balancing physical activity and healthy eating with things like quality sleep and finding ways to relax. RedBrick Track is a tool designed to help you build and reinforce these healthy habits in just a few minutes each day. You ll get ideas for healthy choices that can fit into your daily life, then you simply track the activities you complete each day. You can automatically sync activities with your Track account using compatible devices, apps and programs such as Fitbit and Runkeeper, as well as the new RedBrick Health app, which syncs with Apple Health (iphone users only)

8 RedBrick Journeys The Journeys take really complex topics and lay them out very easily and succinctly; they break them down into really simple, easy terms. Phillip G. Journeys allow you to pick the healthy, inspiring steps that appeal to you, unlocking new levels of healthy habits as you go. Choose from Journeys like Smart Snacking, Go Mediterranean, Fitness to Go, Eat for Energy and dozens more all available online or on your mobile device. Like Track, Journeys can be accessed online and via the RedBrick Health app, which is currently available to iphone and Android users via the App Store or Google Play by entering this activation code: bewellbenefits. One-on-one expert guidance and support With a personal coach, you ll have the benefit of working directly with a certified expert such as a registered dietitian, exercise physiologist or registered nurse, just to name a few. Coaches will work with you over the phone during scheduled calls usually about once a month to answer your health questions, provide support in overcoming obstacles, and help set small goals to work on between coaching sessions

You (and your Capital One Medical Plan enrolled spouse or domestic partner) can each earn up to $350 this year, for a total family reward of $700 by completing healthy activities. Capital One Medical Plan Enrollees Get support from an Anthem Nurse Coach for chronic health conditions or work with an Anthem dietitian. If you have a chronic health condition like diabetes, COPD/Asthma, hypertension, high cholesterol or congestive heart failure, managing your health can be challenging. Anthem s nurses can help you stay healthy, get healthy after an episode of care and live well with chronic illness. Complete a call with an Anthem Nurse Coach and Anthem will let RedBrick know when you ve met with a nurse to earn $15. Visit with an Anthem dietitian and you ll earn $15 toward your 2017 reward.
