1 STUDENT COPY: RETAIN THIS TOP COPY FOR YOUR RECORDS Eastview High School June 11 - July 3, 2012 Eastview will provide a summer school program from Monday June 11 Tuesday July 3 (17 Days) for Eastview High School students wishing to earn credit towards graduation. Students must gain approval for any long term absences (see attached form). Long term absences are defined as missing more than two consecutive days. Courses offered will include: English, Math, Social Studies, Science and Wellness for grades Students completing grade 11 can also prepare for and retake the MCA-II/GRAD Math Exam through their summer school courses. These courses will be for make-up credit only. There is no cost for these courses. The courses will run Monday through Friday with each day organized into three s: Period 1 from 7:30 9:15, Period 2 from 9:20 11:05 and Period 3 from 11:10 12:55. Each is worth the equivalent of one quarter. A student, therefore, could make up as much as 2.25 credits or three quarter length courses if he/she signs up for all three s. Guidelines 1. A student who exceeds three (3) absences will not receive credit for the course and will be dismissed from summer school. Three tardies will equal one absence. Failure to attend class June 11 June 13 (first three days) will result in dismissal from the course. 2. The campus is closed between classes. 3. All regular school rules are in effect during the summer school sessions. 4. Grading will be pass or no-credit. Letter grades will not be given; a P will be issued to students who successfully complete the graduation standard offered in each course. 5. Students are responsible for providing their own transportation to and from EVHS. 4 CLASS SELECTION LIST A summer course may be used to make up course credits or prepare for the required MCA-II/GRAD math exam. For Example: Social Studies (9-10) may be used to make up credits for 9 th grade American Citizenship or 10 th grade U.S. History. English (11-12) may be used to make up credits for English 11 or English 12. Wellness may be taken to make up credits for any of the three wellness courses in grades 9 or 10. Student is performing substantially below performance level for students of the same age on a local achievement test (including not passing on the basic standards /MCA test in reading, math or writing); or Student is behind in satisfactory completing course work or obtaining credits for graduation; or Student is pregnant or is a parent; or Student has been assessed as chemically dependent; or Student has been physically or sexually abused; or Student has experienced mental health problems; or Student has been homeless sometime in the last 6 months; or Student has been excluded or expelled from school; or Student has been referred by the school for enrollment in an eligible program; an alternative program; or Student has limited English proficiency or speaks English as a second language. Summer School Dates absent: from to Reason for Absence: Home Phone Work Phone (Father) (Mother) Extended Absence Basic Rules Any student who may be absent beyond the allowable summer school attendance limit due to a planned family vacation or registration and attendance at another summer credit recovery program (ABE, ALC, etc.) must complete and submit this form with their application. The student must also adhere to the stipulations listed below. We will contact parents/guardians or other programs to confirm dates and/or registration. This form is only a request, not approval for the extended absence. Approval will be given after a parent/guardian is consulted. 1. The student must not be absent or tardy in their summer school courses other than the dates listed above. 2. The student must coordinate with their teachers to complete all assigned work prior to their extended absence. 3. Violation of any school attendance or behavioral rules will cancel approval of the extended absence and normal attendance policies will be enforced.
