5 Student Appointment Form (SAF) INSTRUCTIONS FOR SUPERVISOR AND STUDENT WORKER: 1. Review the following information. 2. Enter the student s work award from the Work Permit. 3. Enter the department name and job title of position. 4. Enter name and SSN of student being hired for this position. 5. Sign and date SAF (Student and Supervisor must both sign.) 6. Return SAF/Work Permit to the Financial Aid Office immediately. The supervisor and student are responsible for complying with the following: 1. Students may not work during scheduled class times. Athletes are not permitted to work during scheduled athletic practices. 2. Students may not exceed the work-study award. 3. Students may not exceed the number of hours per week for the position. For example, the majority of the positions are 15 hours per week or $2160 for the semester. Students and the supervisor must receive permission from the Financial Aid Office to exceed the 15 hour per week requirement. 4. Students may not work more than 20 hours per week when school is in session. 5. The student must submit an accurately completed timecard for each pay period on the dates indicated. 6. Complete job termination notice (available in the Financial Aid Office) when student is no longer working.
