1 Lilly Endowment Community Scholarship Guidelines Lilly Endowment, Inc. designed its Lilly Endowment Community Scholarship Program to raise the level of educational attainment in Indiana and increase awareness of the potential of Indiana s community foundations to improve the quality of life for the state s residents. The Endowment will provide all funding necessary for the awarding in Elkhart County of three, 4-year, full-tuition scholarships, including books and fees, to any accredited Indiana College. One eligible senior will be nominated from each eligible high school in Elkhart County. Students not selected as Lilly Endowment Community Scholars will be awarded the Guy David Gundlach Memorial Scholarship of $20,000 to be paid over 4 years ($5,000 - each year). The scholarship is for tuition only at any 4-year accredited Indiana College. Eligibility Resident of Elkhart County Neither parent with whom the student resides has completed a 4-year degree Accredited HS graduate by June 2018 Student having attained Core 40 or Indiana Academic Honors Diploma or equivalent Must enroll in an accredited Indiana college in the year of high school graduation Planning to enroll in a 4-year (baccalaureate) college degree program in Indiana Timelines August 1, 2017 Application OPEN September 11, 2017 Application DUE (Applications turned in to High School) September 22, 2017 High Schools report top nominees to Community Foundation October 23, 2017 Nominee Interview Day (REQUIRED) October 30, 2017 Community Foundation reports top nominees to Independent Colleges of Indiana December 15, 2017 Community Foundation announces recipients April 28, 2018 Scholarship Awards Brunch (REQUIRED) 1
2 Application Checklist APPLICATION Fill out pages 6-11 of the following application. Print neatly. See Guidelines for completing community, civic, sports, and extra-curricular activities. GRADES Return application to your school counselor to complete page 12 (to include student transcripts). PERSONAL EVALUATIONS Print out pages Name (2) teachers, counselors, or extra-curricular sponsors who know you well & can evaluate your character. (All evaluations are confidential. Please submit in sealed envelopes & signed by evaluator.) ESSAY (Attach typewritten essay to this form) Each applicant will be responsible for personally preparing an essay on a subject selected yearly. The topic for 2018 is: More dangerous than anger and hatred is indifference. Indifference is not a beginning; it is an end and it is always the friend of the enemy. Support or oppose the above quote by Elie Wiesel. Essay may not exceed three double-spaced typewritten pages. The accepted rules of grammar and composition should be used, however points will be awarded primarily on the basis of the overall impact of the essay on the judges. Logic and clear development, as well as the persuasiveness of the argument presented, is most important. The purpose of the essay is to provide the judges with a deeper insight into the character and beliefs of the nominee, as well as the nominee s ability to clearly and cogently communicate with the reader. Note: Essays are graded blind by two Professors selected from Goshen College. PERSONAL STATEMENT Each nominee must include a short statement that should be typewritten and 1-2 pages in length. Please answer the following questions: Describe an example of your leadership experience in which you have positively influenced others, helped resolve disputes or contributed to group efforts over time. If you receive the Lilly Endowment Community Scholarship (LECS), when you graduate from college you will not have the same financial debt as many of your peers. What will you do with that advantage? UNUSUAL CIRCUMSTANCES (Attach typewritten unusual circumstances to this form) Each nominee may include a short statement explaining any family or personal unusual circumstances and how those circumstances have adversely affected the student. (Examples are: medical/health problems for self or immediate family member, single parent or single parent household, blended family concerns, recent tragedies such as death of an immediate family member or assault, physical or learning disabilities, incarceration of parent, and any major adverse family financial circumstances. (Please note: if unusual circumstances have been listed, the committee at its discretion may ask for additional information). INTERVIEW Student selected as his/her school s top nomine MUST be prepared to meet with the interview committee at the CFEC. Interviews will last between minutes and are scheduled for Monday, October 23, Each nominee will be contacted individually to set up an interview time. PHOTO Student selected as his/her school s top nomine MUST be prepared to include one wallet size senior photo. ORIGINAL only. NO copies on paper! 3 GUIDELINES For filling out records of activities and leadership. Only include hours between July 1, 2014 June 30, 2017 COMMUNITY, RELIGIOUS, CIVIC ACTIVITIES Include: Teaching Sunday-school or summer bible-school, youth group service projects, missions trips, 4-H activities, service clubs, volunteering at non-profit service organizations (such as: Faith Mission and Teen Court), volunteer coaching, or other volunteer activities. Be sure to include conferences, workshops, and seminars that deal with personal development, leadership training, and/or service such as Rotary RYLA leadership development. (Please be sure to include positions of leadership) List estimated hours (max 3 hours on a school day and max 6 hours on non-school days) for each year in columns For mission trips or conferences 8 hours per day, not to include travel days to and from. Exclude: Attending youth group, church, bible study, Sunday school, attending sports games, plays etc., as an observer; music lessons or other types of lessons; any activity for which you are paid (list under work experience). Any conference or camp for which you paid to attend. Hours for sports conditioning (working out) outside of official practice times. SPORTS & OTHER SCHOOL EXTRA-CURRICULAR ACTIVITIES Include: Music groups; drama; clubs; Student Council; NHS; academic teams; peer tutoring; teacher assistant, etc.; tutoring or mentoring before, after or during school hours if giving up a study hall or lunch; sports teams after school hours; after school work on school newspaper or yearbook; FFA; FSA; Prom committee or other school committees; band; orchestra after school hours; plays after school hours. Activities such as student government and tutoring that take place during school day may be included, if verified by counselor. You may also include: seminars, workshops, conferences that deal with personal development, leadership training or service. (Please be sure to include positions of leadership) List the estimated after school hours (max 3 hours on a school day and max 6 hours on non-school days), list the number of weeks in which each sport was in session and list the coach for each year in columns Exclude: Any graded activity during the school day such as school assistant if receiving a grade; any paid activity such as giving music lessons (list under work experience); any required attendance activity. Paid camps may NOT be included. 3
4 SCORING GUIDE Informational tool for helping students and guidance counselors complete and review the Lilly Endowment Community Scholarship Application School Extra-Curricular Activities*: One (1) point for every 50 hours of activity. If a student receives a grade for the activity, it is not an extra-curricular activity, but after school hours for that activity may be included. Activities, such as student government and tutoring that take place during the school day, may be included if verified by a counselor (please initial). An additional point (1) may be earned for every year in which they were in a position of leadership (an officer, editor of paper, captain of team, etc.) Community, Religious, Civic Activities*: One (1) point for every 50 hours of participation or volunteer services rendered. An additional point (1) may be earned for every year in which they were in a position of leadership. Note: Students who participate in mission trips or conferences are allowed 8 hours per day, not to include travel days to and from. Work Experience*: One (1) point for every 50 hours of employment. * Only include hours between July 1, 2014 June 30, 2017 Essay: (Blind Alpha Coded) Graded by 2 Goshen College Professors. 7 Student Pledge: 1. If I receive this scholarship, it is my intent to pursue four years of undergraduate study on a full-time basis leading to a baccalaureate degree at an Indiana college. 2. I understand that the total maximum amount of my scholarship is calculated on the basis of my chosen college s tuition and required fees beginning with the school year. 3. To assist with the processing of my scholarship payments each semester or quarter and to avoid late fees, I will forward to the Community Foundation of Elkhart County immediately upon receipt all invoices received for tuition and any eligible fees that may be covered by my scholarship. 4. I will account for the amount of the special allocation spent for required books and required equipment with official receipts and other documentation. I will return to Independent Colleges of Indiana (ICI) any amount of the special allocation remaining at the end of each school year. 5. I agree to notify ICI of any scholarship awards I may receive for tuition or required fees from a source other than the Lilly Endowment Community Scholarship. 6. I will keep the Community Foundation apprised annually by June 1 st of my enrollment and academic status during college, by completing and returning any surveys or forms as may be provided by the community foundation. 7. Upon graduation, I will keep the Community Foundation apprised annually by June 1 st of my education and/or employment status for at least ten years after graduation, by completing and returning an alumni survey or other forms as may be provided by the community foundation. 8. My signature indicates permission for my photograph and name to be published in newspapers and other publications if I am a Lilly Endowment Community Scholarship or Guy David Gundlach Memorial Scholarship recipient. The information in this document is accurate and I give my consent for it to be shared with the Lilly Endowment Community Scholars Program Selection Committee and with Independent Colleges of Indiana (ICI).
