2 Topic IV. CPX Results Report 2015 Presenter Ming Lee, Ph.D. Topic V. CPX Proposal for May 2016: Vote Required Presenters Danny Kahn, M.D., Estebes Hernandez, M.D. MEDICAL EDUCATION COMMITTEE MINUTES: MEETING OF JANUARY 13, 2016 DGSOM participates in the California Consortium for the Assessment of Clinical Competence (CCACC) that oversees the Clinical Performance Exam (CPX) administered at all California medical schools. DGSOM students are required to take and pass the CPX, which is comprised of 8 OSCEs with standardized patients (SPs). Dr. Ming Lee presented the results for the Class of students (30% of the class) failed the CPX with 52 students failing the physical exam component. - The majority of physical exam fails were from omission rather than performing maneuvers incorrectly. - Class performance indicates that students need work in clinical reasoning. What can be done in the clerkships? - Compared to other California schools, DGSOM students had average performance in all areas except patient satisfaction where DGSOM students had lower scores. Over the years, DGSOM SPs have graded more stringently than SPs at other CCACC schools and this may contribute to lower performance. A DGSOM CPX task force is examining student performance and the remediation process. They are using the modified Angoff method to determine if cut-off scores are reasonable. Most CCACC schools fail students who perform two standard deviations below the mean score. UCLA uses absolute Pass scores of 58%-65% on various components. CPX provides a controlled clinical environment to assess competency in core clinical skills including history-taking, physical exams, clinical reasoning, and patient-physician interaction and communication. CPX Remediation Current process - Complete a video review with a clinician, complete an elective from a list of approved courses, and submit one mini-cex physical exam with faculty Informational Vote: A motion was made and seconded. 3 MEDICAL EDUCATION COMMITTEE MINUTES: MEETING OF JANUARY 13, 2016 observation and feedback. - Concerned about variation in the quality of approved electives for remediation - Should Faculty know which students are remediating CPX? Propose providing the information so faculty can help the student. Leadership stipulates, for the record, there is no legal or other prohibition to providing this information to faculty. - Remediation should be direct observation and feedback Next Steps: As this is not changing any requirements, Drs. Noah and Drake will not take to the FEC for approval New CPX Remediation Proposal: - 1 on 1 video review. Until recently only 2 faculty members reviewed videos. The taskforce members are standardizing the process to develop a core faculty group trained in CPX assessment and remediation to provide a plan for each student who fails CPX. - After the review, the students will work on their plan in their electives (all eligible). In place of the elective, we will create 2 SP exam. Each student paired with a faculty buddy from the trained CPX cohort who will directly observe and provide feedback. This will shorten the remediation process and provide consistent quality feedback. After completing remediation the students would then take the CPX retest. Motion: Implement the revised CPX remediation process as described in the MEC minutes of January 13, 2016, immediately (May 2016) for the class of 2017 and to use the Angoff method yearly to guide criterion-based assessment of the CPX. The CPX Taskforce is empowered to explore and consider other remediation plans without having to come back to the MEC for approval. Final_MEC_Minutes_V4_ JS 3 P a g e

Braddock III, M.D., MPH, MACP MEDICAL EDUCATION COMMITTEE MINUTES: MEETING OF JANUARY 13, 2016 Medical Student Council update on Selection Process of Representatives for students in special programs An ongoing discussion on how to incorporate a more inclusive student body to include students from Drew, Prime, MSTP, combined degree program, and other non-traditional degree programs. The proposed additions to MSC constitution are attached. Four representatives (Drew, PRIME, MSTP, and the combined degree program) will share one vote. All representatives can attend MEC meetings, however there will only be one vote. Cynthia He is the MSTP rep.. Drew and PRIME will elect their reps within the next month. An invitation will be put out to the combined degree program These elected representatives will remain in office for the remainder of this calendar year. MSC will be re-electing for next year. Dr. Braddock announced the appointment of Neveen El-Farra, M.D. as Associate Dean for Curricular Affairs in the David Geffen School of Medicine, effective January 1, The first annual Medical Education Report edited by Joyce Fried is published and available in the Dean s Office. 5 There shall be four (4) MEC Representatives representing students in the nontraditional degree programs at DGSOM (e.g. MSTP, PRIME, Drew, MD/MPH, MD/MBA, MD/JD, etc.) The Non-Traditional Degree Program MEC Representatives shall be voting members of the MEC. The four representatives will all share one (1) vote on the MEC The one vote shall be shared among one Representative from the MSTP, one Representative from PRIME, one Representative from Drew, and one Representative from other non-traditional degree programs The Non-Traditional Degree Program MEC Representatives shall be senior (4th year) medical students in Drew, MSTP, and PRIME in good academic standing, and graduating the same year for which they are serving. MSTP, PRIME, and Drew shall elect their respective MEC Representative by a majority vote. The MSC shall solicit applications for and vote on another representative from other non-traditional degree programs. This student must be on leave from the DGSOM curriculum for academic reasons and shall not be a part of MSTP, PRIME, or Drew programs The Non-Traditional Degree Program MEC Representatives will provide input to the MEC to ensure that curricular and educational policy decisions are made with the needs of aforementioned degree program students in mind At least one Non-Traditional Degree Program Representative must be present at each of the MEC s meetings. If multiple Representatives are present at a meeting, the one vote shall be cast in the direction favored by a majority of the Representatives.
