RECOMMENDED MOTION: BE IT RESOLVED, that the Board of Cooperative, First Supervisory District of Suffolk County, has determined that each of the actions being taken through approval of the consent agenda is a Type II action under the State Environmental Quality Review Act (SEQRA) regulations, 6 NYCRR 617.5, which has no significant impact on the environment.

2 B. Recommended Acceptance of Board Meeting Minutes, First Supervisory District of Suffolk County, hereby accepts the Minutes of the August 26, 2003 Regular Board Meeting. C. Recommended Acceptance of Financial Reports The Budget Amendment and Budget Transfers Report for June 2003 and the Budget Summary Report as of June 2003 will be presented to the Board upon completion of our year-end closing activities and independent audit. We expect the year-end closing activities and independent audit to be completed by October 1, 2003 and it is anticipated that these reports will be presented at the October 28, 2003 Board meeting., First Supervisory District of Suffolk County, hereby accepts the following financial reports: Treasurer s Reports for July 2003 Receivables Report as of August 2003 Budget Amendment and Budget Transfers Report for July and August 2003 Schedule of Bills for August 2003 D. Recommended Approval of Bids Bid #003-47: Uniforms The purpose of this bid is to award a contract to supply uniforms for BOCES drivers/messengers and Buildings and Grounds personnel. To ensure the quality of the uniforms, the bid specifications required that the vendors provide samples of each item with the submission of their bid. Only one vendor, Goldman Bros.,, met this requirement. The matter has been reviewed by BOCES general counsel who advise that the requirement to provide samples cannot be waived. The term of contract for this bid is September 23, 2003 to September 23, 2006., First Supervisory District of Suffolk County, hereby awards Bid #003-47: Uniforms to the low bidder meeting specifications, Goldman Bros., of Hicksville, NY. Bid #004-06: Financing for Installment (Lease) Purchase Agreements The purpose of this bid is to award a contract for Financing for Installment (Lease) Purchase Agreements. The specifications called for a bid of an index value that would be applied to the weekly Treasury Note Rate. Koch Financial Corporation was the only

3 bidder meeting specifications. The term of contract for this bid is September 23, 2003 through September 23, 2006., First Supervisory District of Suffolk County, hereby awards Bid #004-06: Financing for Installment (Lease) Purchase Agreements to the low bidder meeting specifications, Koch Financial Corporation. Cooperative Bidding Program Cooperative Bid # : Fuel Oil The purpose of this bid is to award a contract to supply fuel oil. The specifications require a bid price for the delivery of the fuel oil above the Platts Oilgram Spot Barge Price. The recommended award represents a 69% increase over the current delivery charge due to the spiraling cost of fuel oil., First Supervisory District of Suffolk County, hereby awards Cooperative Bid # : Fuel Oil to the low bidder meeting specifications, Sprague Energy Corp. of White Plains, New York for the delivery charge of $.1115 per gallon over the Platts Oilgram Spot Barge Price. E. Recommended Approval of an Agreement with New York Therapy Placement, (Supporting Information Agreement) This agreement with New York Therapy Placement, will provide physical, occupational and speech therapy services to BOCES students when BOCES therapists are not available due to leaves of absence, summer services, etc. This agreement terminates on June 30, BE IT RESOLVED that the Board of Cooperative, First Supervisory District of Suffolk County, hereby approves the agreement with New York Therapy Placement, to provide physical, occupational and speech therapy services to BOCES students during the school year. J. Recommended Approval of the William Floyd Leasehold Space Agreement (Supporting Information Agreement) This leasehold space agreement provides the use of a 21,000 square foot wing in the William Floyd Elementary School for a ten-year period commencing September 1, The total annual base rent for the year is $105,000 plus annual construction costs of $103,320. For the year, the base rent is frozen at the rate unless the Consumers Price Index exceeds three percent, in which case the base rent shall increase by the amount in excess of three percent for that year. For , and years, the rent shall increase from that in effect during and , respectively,

8 by the CPI or four percent, whichever is less. In , BOCES and the district will meet to determine the rent for years 6 through 10 of this lease. Leasehold Space Agreement with the William Floyd School District for the use of a 21,000 square foot wing in the William Floyd Elementary School for a ten-year period commencing September 1, K. Recommended Approval of the Center Moriches Leasehold Space Agreement (Supporting Information Agreement) This leasehold space agreement provides the use of three classrooms in the Clayton Huey Elementary School in the Center Moriches School District for a three-year period commencing September 1, The total annual base rent for the year is $7,425 per classroom. For the year, the base rent is frozen at the rate unless the Consumers Price Index exceeds three percent, in which case the base rent shall increase by the amount in excess of three percent for that year. For , the rent shall increase from that in effect during and , respectively, by the CPI or four percent, whichever is less. Leasehold Space Agreement with the Center Moriches School District for the use of three classrooms in the Clayton Huey Elementary School for a three-year period commencing September 1, L. Recommended Approval of New Casual Employee Title and Rate of Pay At the August meeting, the Board approved agreements related to the creation of a collaborative program for the school year in which students from the East Hampton School District can enroll in an exploratory vocational program based at the Ross School. Because of the small number of students enrolled in this program, as well as the extremely limited instructional format and the remote location, it is most appropriate to utilize hourly employees for the instruction and administrative coordination related to this program. casual employee titles of Daily District Administrator at the compensation rate of $75 per hour and District Industrial Arts Instructor at the compensation rate of $55 per hour. M. Rescission of Appointment and Appointment of the Internal Claims Auditor Mrs. Carol Euring, who was appointed by the Board as the Internal Claims Auditor at the July Organization Meeting, has been promoted to another position within the Business Office. Serving in the new position will conflict with the responsibilities of an internal claims auditor.

9 , First Supervisory District of Suffolk County, hereby rescinds the appointment of Mrs. Carol Euring as Internal Claims Auditor effective September 23, BE IT FURTHER RESOLVED that the Board of Cooperative, First Supervisory District of Suffolk County, hereby appoints Mrs. Patricia Bombard as Internal Claims Auditor for effective September 24, N. Recommended Authorization to Develop and Conduct a Building Survey by Wiedersum Associates, PLLC (Supporting Information Proposal), First Supervisory District of Suffolk County, hereby authorizes Wiedersum Associates, PLLC to develop and conduct a detailed survey of all BOCES owned and leased buildings at a fee of $78,000. O. Recommended Approval of Casual Employee Titles Rates of Pay The Executive Director of Human Resources recommends increasing the rates of pay for these three casual employee service categories to allow us to stay competitive in reimbursement for these services. These rates have not been increased in five years. RECOMMENDED MOTION: "BE IT RESOLVED that the Board of Cooperative increase in the casual employee rate of pay for Psychological Evaluations from $325 per evaluation to $440 per evaluation, for Evaluations from $230 per evaluation to $250 per evaluation and for Social History Evaluations from $90 per evaluation to $100 per evaluation.
