1 Simi Valley Unified Universal Scholarship Packet There are 3 parts to this Universal Scholarship package: 1. Instruction page 2. Scholarship Opportunities 3. Application (2 pages) Attached is a list of the Simi Valley Unified School District scholarship opportunities available from local organizations/clubs and individuals. With this single application, you can apply to any or all of the scholarships included in this packet. INSTRUCTIONS: Carefully read the description of each scholarship and indicate with a checkmark the scholarship(s) for which you wish to apply. Keep the list of scholarship opportunities for your reference. Fully complete the Universal Scholarship Application (type in the boxes). Be advised that the scholarship committees are looking to see if you can follow instructions correctly. Incomplete application packets will not be accepted. Fulfill the additional needs of each individual scholarship. Follow the directions! DO NOT put the application in an envelope or folder; staple the scholarship application pages in the upperleft hand corner. Put the application together in order as listed below for each scholarship: UNIVERSAL SCHOLARSHIP APPLICATION 2. PERSONAL RESUME: Please include the following for your 9 th -12 th grade timeframe: Education Awards/Certificates - List award, describe and indicate year received. Extracurricular Activities - Activities, clubs, and sports in which you participated. Indicate leadership/officer positions held, responsibilities, and number of hours/ year(s) of participation. Community Service - Include charitable, civic, and community organizations for which you volunteered. Indicate responsibilities and number of hours/ year(s) of participation. Work Experience - List any job positions you have held & responsibilities of each job. 3. SUPPLEMENTAL MATERIALS - as required for each scholarship (i.e. essay, PTSA membership card, etc). (Put your name on top right-hand corner of any essay you are submitting) 4. TWO SIGNED LETTERS OF RECOMMENDATION - You can be recommended by teachers, counselors, employer or family friend, NO family members. No letters of recommendation will be accepted. (Letters must have a signature on them; copies are ok). 5. CURRENT UNOFFICIAL TRANSCRIPT Available from the registrar (NOT an aeries report)

2 TO NOTE: YOU will need to make a copy of each packet (i.e. transcripts, essays, and letters of recommendations) for each scholarship. (Applying to 5 scholarships? You should submit 5 COMPLETE packets, making sure to change the name for each scholarship!) DO NOT use a cover letter, paper clips, folders or envelopes. Staple in the upper-left hand corner. Turn in the completed packets to the College Career Center by 3:15 P.M. on April 16th. NO late applications will be accepted for any reason! You may turn in applications before the deadline. 3 Simi Valley Unified School District Universal Scholarship Opportunities To the student: Complete the requirements for EACH scholarship that you wish to apply. See instruction page for information on how to put each scholarship packet together. You can keep the list of scholarships for your reference: American Association of University Women Graduating female senior with a 3.0 GPA planning to attend an accredited college or university in the fall of Student must be a resident of Simi Valley and attend a Simi Valley Unified School District school. Please indicate if you are related to a member of a branch of AAUW (include name, relationship and branch). Essay: Describe a woman who has inspired you and helped provide some direction in your future plans. Elaborate on your future goals and how you foresee yourself influencing others. Henry Lauer Memorial Scholarship Scholarships are awarded to Simi Valley High School seniors. This scholarship is designed to give students who may have struggled academically in the past an opportunity to achieve their educational goals after high school. It is available to all seniors who have worked hard to make that change, regardless of GPA, who are graduating in 2019 and plan on attending an accredited 2-year college, 4-year University, or other career training program. All students deserve a chance to excel in life, and we hope that this scholarship, dedicated to their beautiful son, will afford all students the opportunity to excel in life. Please submit a word essay identifying the following information: 1. Challenges that you have overcome in your life. 2. How those challenges have shaped your life and enabled you to set clear goals for your future. Madera Elementary School PTSA Must have attended Madera for at least two years; student must be a current member of his/her local high school PTSA and is pursuing a post-high school education. Along with a membership card, please submit an essay including the following information: 1. What are your plans following graduation from high school? Please note the college you plan to attend and what you expect to study. 2. What extracurricular activities have you participated in? Please note any leadership roles you have held and any volunteering you have done. 3. What is your favorite memory of your years at Madera Elementary School? 4. What advice would you give to a current Madera Elementary School student to prepare him or her for success in middle school and high school? 5. What do you believe most sets you apart from your peers in applying for this scholarship? Simi Valley Education Foundation Scholarship Graduating senior from a high school in Simi Valley Unified School District (Royal, Santa Susana, Simi Valley, Apollo, and Monte Vista) that will be attending an accredited trade school with certification within SVUSD, two- or four-year college. Include a one-page typed personal essay submitted summarizing your hopes and goals for the future, plus a summary of your leadership and work experiences (both paid and volunteer) at your high school and in the community.

4 Simi Valley Friends of the Library Graduating senior with a minimum 3.0 GPA and must be planning to attend a vocational school, two- or four-year college; must have completed at least 25 hours as a Simi Valley Community volunteer from June 2018 to the present. Include an essay not to exceed 400 words summarizing how a solid foundation in reading and/or use of the Library has affected your life and can further your educational goals. Rotary Club of Simi Sunrise: College/University Scholarship Offered to graduating seniors who plan to attend a college or university. Please note, for the parent information page of the application, only parent name and contact number are required. Applicants must respond to the following: 1. Explain how contributions you have made to the community relate to Rotary International s values of honesty, truth and responsibility, fairness and equality for all people, building goodwill and better friendships and helping to ensure that your actions are beneficial to all. 2. Describe any challenges you encountered in high school and what you did to meet those challenges. 3. Describe your college and/or career goals. What do you see yourself doing after you complete your education? 4. Provide an Applicant Statement of no more than 300 words. Tell us why you believe you should receive a Rotary Club of Simi Sunrise scholarship. You may want to include information about you, your values and beliefs, your dreams and goals for the future or how you hope to make a positive difference in the world. _ Rotary Club of Simi Sunrise: CTE Scholarship Offered to graduating seniors who plan to attend a career technical education (vocational) school or program. Please note, for the parent information page of the application, only parent name and contact number are required. Applicants must respond to the following: 1. Explain how contributions you have made to the community relate to Rotary International s values of honesty, truth and responsibility, fairness and equality for all people, building goodwill and better friendships and helping to ensure that your actions are beneficial to all. 2. Describe any challenges you encountered in high school and what you did to meet those challenges. 3. Describe your college and/or career goals. What do you see yourself doing after you complete your education? 4. Provide an Applicant Statement of no more than 300 words. Tell us why you believe you should receive a Rotary Club of Simi Sunrise scholarship. Simi Valley PTA-PTSA Council Graduating senior with a minimum 2.5 GPA who is a member of their PTSA by March 15, 2019 and has applied to a two- or four-year college or vocational school. Include a copy of your PTA/PTSA membership card and TWO one page essays (12 point font). Your first essay should address how your involvement in school and community activities helped you develop as a person. Your second essay should address what impact has another volunteer made on your life and why.

5 Simi Valley Education Foundation Endowment Program Scholarship offers awards to students who cannot afford to fund their own education and show a strong desire to succeed by educating themselves. The intent is that these scholarships will offer an opportunity or turning point in a young person s life that inspires them to go on to be an asset to our community. The recipients should be students who have tremendous potential, but may not have outstanding grades. The committee will be looking for individuals who have shown significant improvement during his/her high school career. The Endowment Program provides a two-year scholarship in the sum of $1,000 for the first year and after verification of the successful completion of classes, the student can apply for the second year allotment. Requirements for this scholarship include: Available to present and past Simi Valley public high school graduates. Must be planning to attend an accredited trade school with certification within SVUSD, twoyear college, or four-year college on a full time basis (minimum of 12 semester units) or participate in a Certificate Program at the Simi Institute for Careers and Education. The recipients must have a financial need. In addition to the SVUSD Universal Application, a one page typed personal essay submitted summarizing your hopes and goals for the future, plus a summary of your leadership and work experiences (both paid and volunteer) at your high school and in the community. Also include your need for scholarship support, including any information you believe the Scholarship Committee should have, such as family income, estimated educational expenses, and any other unusual circumstances that affect your financial situation. At the end of each school year in which a scholarship was provided, you will submit a one page typed summary of how the monies provided made a difference in supporting your education and share your story about how furthering your education has made a positive difference in your life. Use the following link: Simi Valley High School PTSA This year the SVHS PTSA has a total of $4, in scholarship money to be awarded to qualified seniors. The scholarship is to be used toward tuition, books, or other expenses at the college, university, or vocational school of the recipient s choice. Each applicant must: have been a SVHS PTSA member in good standing during their junior and senior year as of 12/31/18; be a SVHS senior with a minimum GPA of 2.0; be in the process of enrolling in or have been accepted by a two-year junior college, a four year college or university, or a vocational school; have shown initiative through involvement in school/community programs, including providing service to SVHS and/or the community. Complete packet must include a copy of your PTSA membership card and an essay. Essay Questions (300 word minimum - typed). Please answer two of the three options. Question 2 (in bold) is required. 1. Tell us about the area of your life (academic, social, etc.) in which you feel you have grown the most during high school, focusing on how you were motivated to overcome any adversity or challenge. Highlight the individuals, organizations or programs at SVHS or in the community which had the most impact on your success. 2. Describe your post-high school plans and how they align with your interests and strengths. Outline the steps you have taken already and will take over the next 2-4 years to achieve your goals. How much of community service/involvement plays into your plan? How would a scholarship help with this plan? 3. Tell us how you have served either the community of Simi Valley and/or the community of SVHS (through PTSA/club involvement) over the last four years. How has that changed you in terms of how you look at yourself and your post-high school goals?

6 Simi Valley Women s Club Graduating senior planning to attend a vocational, community or 4 year college. Submit a 200 word essay describe what hardships you ve encountered at school and how you overcame them and why you deserve the scholarship. Donald D. Todd, Sr. Scholarship Fund This annual scholarship in the amount of $1,000 is in honor of the late Donald D. Todd, Sr. During the latter years of his life, Don served as a campus supervisor at Simi Valley High School. Don enjoyed conversing with the students he was supervising. In particular, he looked to mentor the student athletes because he could relate to their love of sports. While Don encouraged students to do their best while competing, he also was sure to push students in their academic success. As a businessman and a father of multiple college graduates, Don knew that these students would not go anywhere in sports without earning high grades. This scholarship should be awarded to a Simi Valley High School Student who is a student athlete, who has kept a 3.0 G.P.A. during all four years of high school, who has marks of outstanding and/or satisfactory in citizenship, and who has received a letter of admission from a four year university. In addition to the SVUSD Universal Application, please submit a 1-2 page typed personal essay introducing yourself. Describe your experience as a student-athlete, and how you plan to use this scholarship money to further your education. Students must be able to produce or complete documentation to meet the following eligibility criteria to qualify for this scholarship: Be a student athlete Have been accepted to a four-year university Have a G.P. A. of 3.0 or higher Have excellent scores in citizenship: Outstanding and Satisfactory Student has participated in extracurricular events at school or in the community beyond sports (i.e. student government, music, debate team, logged volunteer hours, Boy Scouts, etc.) Valley View Middle School PTSA Must have attended VVMS for at least two years; student must be a current member of his/her local high school PTSA and is pursuing a post-high school education. Along with a membership card, please submit an essay including the following information: 1. What are your plans following graduation from high school? Please note the college you plan to attend and what you expect to study. 2. What extracurricular activities have you participated in? Please note any leadership roles you have held and any volunteering you have done. 3. What is your favorite memory of your years at Valley View Middle School? 4. What advice would you give to a current Valley View Middle School student to prepare him or her for success in middle school and high school? 5. What do you believe most sets you apart from your peers in applying for this scholarship?

7 Mary Ann Mother Mary Todd Scholarship Fund This scholarship is in honor of, Mother Mary and the Pioneer Spirit. Mother Mary has served as a campus supervisor in SVUSD schools for decades. She is an example of what it means to be a hard worker. She tirelessly gives an endless amount of energy, drive, and willingness to give 100% of herself every day. In addition to working hard, Mary has always been someone who is willing to lend a helping hand to those in need. Whether it be giving a student money for lunch, being the one person who listens, or giving students words of encouragement, she is always there to give others that little lift that will help get them further. It is in that spirit that this scholarship is made available to a Simi Valley High School student in the amount of $1000. The recipient of this annual scholarship should be students who work hard, who are respectful towards themselves and others, and who are not afraid to put in the extra work to get the maximum results. In addition to the SVUSD Universal Application, please submit a 1-2 page essay in which you first introduce yourself, explain why you are pursuing and deserving of this scholarship, and how you plan to use the scholarship money to further your education. Furthermore, explain how you embody the Pioneer Spirit. Essay must be submitted in hard copy form along with other paperwork. The essay will be read by Mary and/or a member of her family. Students must be able to produce or complete documentation to meet the following eligibility criteria to qualify for this scholarship: Have a serious intent on furthering their education; be it a four-year university, community college, vocational/trade school Have a G.P. A. between 3.0 to 3.5 Have excellent attendance throughout high school career Have excellent scores in citizenship These additional factors will be used if more than two students apply. Student has participated in extracurricular school events, including but not limited to sports, music, marching band, drama, student government, volunteer Letter of Acceptance to a credible educational institute A COMPLETE PACKET NEEDS TO BE SUBMITTED FOR EACH SCHOLARSHIP THAT YOU ARE APPLYING FOR!
