3 Invitation to participate The Royal College of Emergency Medicine (RCEM) is pleased to announce the launch of The Emergency Medicine Foundation Programme and to issue an invitation to international healthcare services to participate in a pilot of this one-year training programme. This new curriculum will provide a quality assured training of key professional and clinical competences in Emergency Medicine at a pre-specialist level. It is aimed at those who have completed residency and wish to develop skills in emergency care, including those who may wish to progress towards further specialist training and membership and fellowship exams. Participation in the pilot of this programme will include delivery of a quality assured training, of key clinical and professional competences, within a structured educational framework that includes supervision from senior clinicians delivered within a local learning hub. Services will have access to a fully developed curriculum, with tools and guidance for training and assessment and associated learning and development resources. This will be complemented by a training programme for clinical and educational supervision. This is an extremely exciting opportunity for both services and clinicians to further develop competence in emergency medicine and standardise care delivery. We now invite services to submit expressions of interest in piloting this programme. 4 Introduction to programme The Royal College of Emergency Medicine (RCEM) is committed to supporting the development of Emergency Medicine at a Global level. As part of this commitment the International Education Committee within the College has developed a training curriculum, aimed at providing registered medical practitioners at pre-specialist level with a demonstrable period of study and training in Emergency Medicine. The Emergency Medicine Foundation Programme (EMFP) will be a one-year course that has a defined and evidence-based curriculum with key competences, both professional and clinical, that support services to provide care that is safe, effective and responsive to population needs. Guidance on methods of learning and assessment will be available, along with additional supportive training tools and resources. The pilot To ensure that the curriculum meets trainee and service requirements, we will be inviting non-uk health care organisations to enroll in a pilot for one year. A draft curriculum has been developed through discussion by the RCEM International Education Committee, which has varied membership including clinicians who have trained abroad. The UK Core syllabus has been utilised but this is not a re-drafting of this three-year curriculum, but a bespoke piece of work that has been shaped to ensure it is suitable for trainees at two to three years post undergraduate training. It will provide core skills, that can be can be completed within a year, can be assessed in a standardised and fair way and will equip trainees with key skills to ensure safe and entrusted practice. Delivery of training as outlined in curriculum with work place-based assessment, online learning and development of self-reflective practice To deliver the proposed curriculum participating services will be asked to commit funds and other resources, including recruitment of senior clinicians to provide educational and clinical supervision to supervise and assess trainees. Each training site will have representation within a faculty group at Hub level, that will oversee the delivery and assurance of this programme. In addition, representation may be sought from management, human resources or administrative staff as well as trainees to also join the Faculty group. Further details about Learning Hubs and Faculty Groups will be available on the RCEM website ( Criteria for organisation inclusion Inclusion within this one-year programme will be subject to assurance that services meet key criteria in terms of access to adequate exposure to common emergency presentations, within a safe and supervised work environment. The programme would only be delivered in centres approved by the College but recruitment into the programme would be the responsibility of the training centre. It

5 is anticipated that the programme would launch initially in India and Pakistan before being rolled out to other interested areas. Criteria for trainee involvement To qualify for inclusion within this one-year programme, trainees must: be registered medical practitioners, with internationally recognized medical qualification be working within Emergency Medicine during the pilot period (August July 2020) not be participating in another training programme during this period Overview of proposed curriculum and it s assessment The curriculum will encompass 20 modules, 18 of which will be compulsory for all pilot sites with two additional optional modules, to allow hubs, services or individual trainees to adapt the curriculum to meet professional as well as local population requirements. Content will be split into the following categories: Generic professional competences e.g. obtaining valid consent, effective communication and handover. The clinical content will be based on the management of common EM presentations, suitable for clinicians at this level in their professional career, and with presentations based on case mix and pathologies most relevant to the local setting. Clinical modules will encompass resuscitation skills, initial management of major trauma and common acute presentations. The learning methods would include self-directed learning, e-learning modules, courses, classroom learning, simulation and experiential learning in the ED. Assessment would most likely be via workplace-based assessments, completion of e- learning and a final summative assessment. Conditions of contract and cost Each organisation will be asked to contribute a sum for overall governance and assurance of programme, through development of a learning hub. A payment of 25,000 per organisation (to cover up to three hospital sites and 30 trainees) will be required at start of the programme. This fee will be used to support the following: Delivery of a bespoke curriculum providing and assuring the competency in key knowledge, skills and behaviours in Emergency Medicine at a level suited for pre-specialist trainees. Provision of training resources and assessment tools mapped to Curriculum for trainees. Credentialed proof of learning.

6 Provision of online facility for collating evidence of training attainment (eportfolio) to be used for appraisal. One-year membership to RCEM UK for trainees and supervisors. In-country induction tools for Supervisors. Ongoing support and training for supervisors via webinars, online. Support in setting up Learning Hub. Appraisal of Supervisors within Hub. Beyond the pilot year costs for healthcare providers would be considerably reduced and would support membership costs for new trainees and trainers, annual appraisal and biannual visits. A formal agreement will be required between the participating organisations and RCEM, which will encompass the following: Completion of initial assessment of service suitability, in terms of activity, supervision, infrastructure and governance. Assurance that senior clinicians will be willing to participate, attend training and provide teaching, supervision and assessment for trainees participating. Assurance of trainee suitability to participate. Assurance of financial commitment to project. Next steps Organisations that are interested in participating in the pilot should ensure that there is local enthusiasm for this project and that both trainees and potential supervisors are fully onboard and able to commit the time to this work.
