Act 1956) A multi-campus university with A grade accreditation by NAAC Amrita Institute of Medical Sciences A super specialty tertiary care hospital accredited by ISO , NABL & NABH AMRITA SCHOOL OF PHARMACY RULES AND REGULATIONS applicable for admission to M Pharm, Pharm D (P.B), M.Pharm Ph.D Integrated and Pharm.D (PB) Ph.D Integrated Programmes 2016 AMRITA SCHOOL OF PHARMACY a constituent unit of AMRITA VISHWA VIDYAPEETHAM a Deemed University established under Section 3 of the UGC Act of 1956 conducts undergraduate and Post graduate programmes in Pharmaceutical Sciences leading to the award of B Pharm, Pharm D, Pharm D (PB) (Post Baccalaureate) and M Pharm degrees. These programmes are conducted in the Health Science campus, Kochi, Kerala, India. The Rules and Regulations explained here are applicable to M.Pharm and Pharm. D (Post Baccalaureate) programmes. Admission to these programmes are made on the basis of marks secured in qualifying examination and performance in the personal interview. Admissions are governed by the Rules and Regulations enumerated hereunder. All candidates who take admission in Amrita School of Pharmacy are deemed to have fully read and accepted the conditions herein contained. VERY IMPORTANT Management of the AMRITA VISHWA VIDYAPEETHAM reserves the right to change any of the conditions enumerated herein for the purpose of complying with any of the regulations of the University Grants Commission/ Pharmacy Council of India or any other competent Authorities. 1. DURATION, ELIGIBILITY & No. OF SEATS M Pharm Ph.D Integrated Pharm D (PB) M.Pharm Ph.D Integrated Programme Duration Eligibility No. of Seats 10 seats each in a) Pharmacy Practice 2 years (Hospital & Clinical Pharmacy) ( 4 Semesters) b) Pharmaceutics c) Pharmaceutical Chemistry d) Pharmacology Pharm.D (PB) Ph.D Integrated 3 years * (2 years of studies plus one year internship or residency 2+2years 3+1years (i) B.Pharm degree form institutions with PCI s approval (ii) Not less than 50% for all the subjects of B. Pharm from second year onwards (iii) GPAT score preferred for M.Pharm admission 10 seats 3

4 M Pharm: Those students with GPAT score are eligible for a scholarship from the All India Council for Technical Education. Current amount of scholarship is Rs 8000 per month. Pharm D (P.B) * Period of three years duration of Pharm D (PB) is divided into two phases as under: Phase I Consisting First and Second Academic Year Phase II Consisting of Internship or residency training during third year involving in specialty units. It is the phase of training where a student is exposed to actual Pharmacy practice or clinical pharmacy service, and acquires skill under supervision so that he or she may become capable of functioning independently. In M.Pharm Ph.D programme, students can opt for either M.Pharm(2yrs) M.Pharm &M S by Research (3 yrs) or M.Pharm Ph.D (4 yrs) 2. CHOICE OF PROGRAMMES Candidates may please note that the personal interview is common for selection of students for M.Pharm and Pharm.D (PB) programmes. While the candidates are welcome to indicate their choice by selecting the appropriate option, actual allotment to the two programmes and different specializations of M.Pharm will be on the basis of marks secured in qualifying examination and performance in the personal interview. Choice may be indicated by selecting M.Pharm or Pharm.D (PB) or if you wish to consider for both the programmes, indicate your choice by selecting the option both. 4. TELEPHONE NUMBERS & ID (VERY IMPORTANT) It is advisable to mention more than one telephone number for contacting the candidate or his / her parents. Many times important messages are given through telephones and by e mail. While the Institution will make all reasonable efforts to contact the candidate or his / her parents, if the numbers are not reachable / e mail ID is not valid, Institution will not be responsible. Therefore candidates should ensure that two or more easily accessible numbers and a valid E MAIL ID are mentioned in the application. 5. ONLINE APPLICATION I. Online Application forms are available at AMRITA web site: heathsciences ii. Application fee can be paid through online Net banking/credit card/debit card using iii. Bank charges if any will be borne by the candidate. iv. Application fee is non-refundable and no correspondence on the subject will be entertained.. v. Keep a printout of the confirmation page of the application form for future reference. AGE 6. CURRICULUM Age limit shall not be below 21 years as on Updated curriculum approved by Amrita Vishwa Vidyapeetham University and in line with the directives of Pharmacy Council of India and recent trends in the profession. 4

5 7. SELECTION PROCESS: Selection of candidates is on the basis of their B.Pharm marks and personal interview. For this purpose B.Pharm Marks list is most essential. Candidates must that the marks lists are submitted days prior to the date of interview. DATE OF THE INTERVIEW Date: 15 th July 2016 (Friday) ensure at least seven Eligible candidates will receive an intimation regarding the date of Interview. They should report at the Office of Dean of Research at least by 9.00am. 9. INTERVIEW: a. Candidates should report at Dean Research Office at least 60 minutes before the commencement of the Interview. b. If GPAT qualification certificate is not enclosed, weight age will not be given for GPAT score. 10. DECLARATION OF RESULT a. Results of interview are likely to be published within two to three weeks. Selected candidates are required to make a payment of Rs 20,000/- and balance of fees for the first year should be paid on the date of admission which will generally be within ten days from the date of interview.. 8. 6 11. ADMISSION a. While reporting for admission, please carry all the relevant certificates/documents in original as listed below. i. B. Pharm Degree certificate; Mark lists, course certificate and GPAT certificate (if qualified). II. High school /Higher Secondary Certificate in proof of date of birth. III. Transfer Certificate f r o m the Institution last studied. IV. Migration certificate. V. Demand draft covering the tuition and other fees the details of which will be intimated by the Office of Dean of Research. VI. Demand Draft shall be in the name of Amrita School of Pharmacy payable at kochi b. These original documents will have to be submitted at the time of admission. These documents will be kept with the Institution till completion of the programme. c. I n the matter of allocation /admission of candidates, decision of the Chairman Admissions shall be final and binding. 12. VERIFICATION OF DOCUMENTS All a d m i s s i o n s are s u b j e c t to verification of the original certificates and documents of the c a n d i d a t e. The decision of the D e a n R e s e a r c h Amrita Vishwa Vidyapeetham who is the chairman of Pharmacy Admission regarding the eligibility of any applicant shall be final & binding. Rs.25,000 will be payable as administrative expenses. b. In addition to the above such students will have to pay tuition fees, host el, mess and other fees proportionately for the period they remained on Rolls of the School. c. For the purpose of calculation of proportionate recovery part of a month will be considered as a full month. d. Subject to the above conditions, for those who e f leave the programme after commencement of classes but before closure of admissions tuition fees will be refunded provided the Institution could fill the vacancy. As per the regulations of the Pharmacy Council of India, admissions will have to be closed by 30 th September, of the year. After closure admissions the seat if vacated and not filled up, such seats will go vacant for the entire duration of the programme. Therefore those who leave the programme after closure of admissions will be liable to pay liquidated damages equivalent to the tuition fees for the remaining period of the programme. For candidates shifting to another school/college of Amrita Vishwa Vidyapeetham deductions will be restricted to Rs.10,000/- (Ten thousand administrative charges of only) as g. Candidates will be deemed to have withdrawn if he/she remains absent for more than 30 (thirty) days from the date of commencement of the classes. 13. CANCELLATION OF ADMISSION AND REFUNDS a. For candidates w h o have joined for a programme but opt to discontinue the same any time before completion of the programme, an amount of 6

MEDICAL INSURANCE All students will be covered under group medical insurance scheme, within stipulated limits. 15.ATTENDANCE A minimum 90% attendance is compulsory for appearing for the university examinations. 16. DISCIPLINE The school, hospitals and hostels are parts of Mata Amritanadamayi Math, the parent trust and therefore, rules and regulations as prescribed should be strictly followed. a. Students staying in hostels have to comply with hostel rules. b. Smoking and alcoholic drinks are totally prohibited. c. Students are expected to dress up neatly and decently. Jeans, Baggies, T-Shirts and the likes are not permitted to be worn inside the hospital or school. d. There will be regular value oriented classes/ discussions in the campus. Students are advised to participate in these programmes. This is an institution meant for higher learning and the objective is not only to impart education but also to build the character of the students and transform them into better human beings. e. A student handbook will be issued at the time of admission which will contain all the rules and Regulations in detail. The students shall strictly abide by them. f. Students are expected to observe absolute discipline in their conduct during the entire period they spend in the Institution. Failure to observe absolute discipline will invite appropriate action. 17. JURISDICTION Courts situated in Kochi, Kerala only will have jurisdiction over disputes if any arising in the matter of applications and or admission to the programmes covered under these Rules and Regulations. 18. SETTLEMENT OF DISPUTES In case of any dispute in the interpretation of any of the conditions included in this booklet or in any other matter related to admission to M. Pharm in Amrita School of Pharmacy at Kochi, The decision of the D e a n R e s e a r c h Amrita Vishwa Vidyapeetham who is the chairman of Pharmacy Admission regarding the eligibility of any applicant shall be final & binding. 19. PREVENTIVE MEASURES AGAINST RAGGING 'Ragging' means doing of any act by disorderly conduct to a student of an educational Institution, which causes or is likely to cause physical or psychological harm or raising apprehension or shame or embarrassment to that student and includes teasing or abusing or playing practical Jokes or causing hurt to such students or asking a student to do any act or to perform something which such student will ordinary course be willing to do. not in the In case, the applicant for admission is found to have indulged in ragging in the past or if it is noticed later hat he/she has indulged in ragging, admission may be refused or he/she shall be expelled from the educational Institution. Anti-ragging committees and Anti-ragging squads will be formed to take effective measures against ragging and they will adhere to the stipulations and effectively monitor and com ply with the directives Every student of the institution and his/her parents, or guardian is required to submit a combined undertaking at the time of registration I admission in bed format (which will be provided by the Institution) which is mandatory for registration/ admission. Anti ragging regulations of the University Grants Commission form part of the terms of admissions and are made available to all candidates and their parents for information and compliance. 7
