1 ADMISSIONS IN FIRST YEAR MBBS AND BDS AT Dow Medical College (DMC) & Dr. Ishrat-ul-Ebad Khan Institute of Oral Health Sciences (DIKIOHS) (Session 2012) DOW UNIVERSITY OF HEALTH SCIENCES, KARACHI Applications are invited for admissions in the category of open merit and UEDP local for MBBS (session 2012) at Dow Medical College (DMC), AND BDS (session 2012) at Dr. Ishrat-ul-Ebad Khan Institute of Oral Health Sciences (DIKIOHS) from 3rd Sept till 17th Sept. 2012, (5:00 pm) ENTRANCE TEST WILL BE HELD ON SUNDAY 7th October 2012

2 ELIGIBILITY CRITERIA for I Categories of MBBS Course at DMC Open Merit (DUHS) Merit Cum choice (from DUHS to LUMHS(Hyderabad), PMC (Nawabshah), & FJMC (Lahore)) UEDP - Local (Self finance) (DMC) and Disabled seats (DMC) AND II Categories of BDS Course Open Merit UEDP Local Matric / O Level / equivalent examination certificate from Karachi.. Intermediate Science / A Level / equivalent examination from Karachi. Minimum marks of 60% aggregate in intermediate Science (Pre-medical) / A Level / equivalent examination. Candidate s domicile certificate of district Karachi. Candidate s permanent residence certificate (PRC, on Form-C) of district Karachi. Candidate s CNIC (Computerized National Identity card ) or B form (if he/she is under 18 year of age). Candidate s father domicile of Sindh province on form P-1. Father s National identity card of NADRA with permanent address of Sindh.

3 ELIGIBILITY CRITERIA For UEDP - Foreign Naionals AND UEDP Overseas Matriculation, O Level / equivalent examination from outside Pakistan. Intermediate, A Level / equivalent examination from outside Pakistan. The candidate must have passed biology, chemistry and physics / maths or any other science subject in A Level / equivalent examination. As per PM&DC rules candidates shall have to produce valid TOEFL / IELTS Certificate with a minimum score of 500 / 5.5 respectively, (if applicable.) IBCC equivalence certificate with minimum 60% marks OR SAT II min score 550 each in biology, chemistry and any other science subject. Proof of being overseas Pakistani, or foreign National. CNIC from NADRA issued as overseas Pakistani to candidate s father. Ineligible candidates will not be issued admit card for the entry test and the payorder submitted will not be refunded.

4 DOCUMENTS To be submitted for Categories of Open Merit (DUHS & LUMHS), UEDP Local, (DMC, DIKIOHS, PMC, FJMC & Disable) Blue and Pink application forms, alongwith following documents must be attached in one ordinary office file. 1) Birth Certificate of the Candidate. 2) Matric Certificate / O Level Equivalent examination certificate from Karachi 3) Mark sheet of Matric /transcript O Level / Equivalent Examination. 4) O Level equivalence / IBCC certificate (if applicable). 5) Interscience / A-Level marksheet or transcript from Karachi. 6) Provisional certificate from School (sample certificate is attached). 7) Provisional certificate from college. (sample certificate is attached). 8) Hafiz-e-Quran certificate (if applicable). 9) Certificate of disability (if applicable). 10) Payorder of Rs.500 (Five hundred) as application processing fee is non refundable. 11) Candidate s Domicile Certificate of district Karachi. 12) Candidate s Permanent Resident Certificate (PRC, on form C) district Karachi. 13) Candidate s NADRA National Identity Card (CNIC) or B form, NADRA (if he/she is below the age of eighteen years). 14) Father s Domicile of Sindh Province on form P-1. 15) CNIC of father, issued from NADRA with Permanent address of Sindh. 16) Six recent passport size photographs (with full face exposure of both males & females) with the name, 3 attested from front & 3 attested from back. 17) Admit card for the MBBS/BDS entrance test, attached with the application form (must be filled by the candidate). No marksheet whether orignal or revised will be accepted after 17TH SEPT Candidates are also required to fill online forms available on DUHS website in addition to Blue and Pink apllication form, this is mandatory. The payorder, CNIC copies and photographs must be attached to the Specified paper attached with application form. Read the important instructions sheet given to you with the application form, before submitting documents. Do NOT submit the instruction sheet with your documents. IMPORTANT Any candidate found using Domicile / PRC of more than one district / place in Pakistan will be disqualified for admission at any college of Dow University or in any other medical institution of Sindh Province and the case will be referred to the concerned authorities for investigations. The domicile certificate of mother (Sindh Province) shall only be accepted in case where father has expired (and no domicile certificate was issued to her / him) or there has been remarriage / separation, for which a valid document will be needed.

5 DOCUMENTS To be submitted for Category of UEDP Overseas and foreign Nationals at DMC & DIKIOHS Following documents must be submitted attached in ordinary office file. 1) Birth Certificate 2) Blue and Pink Colour application forms 3) Matric / O Level / Equivalent examination Certificate (from outside Pakistan). 4) Matric / O Level / Equivalent Examination mark sheet or transcript. 5) O Level / IBCC equivalence certificate. 6) Inter Science / A Level / Equivalent examination transcript. (from outside Pakistan). The candidate must have passed biology, chemistry and physics / maths in A Level / equivalence examination. 7) Certificate / diploma of A Level / equivalent / Inter Science. 8) SAT II min score 550 in biology, chemistry and physics / maths. or IBCC equivalence certificate with (minimum 60% marks). 9) Father s CNIC from NADRA issued as overseas Pakistani. 10) Proof of being overseas Pakistani. 11) Six recent passport size photographs with the name, 3 attested from front & 3 from back. After verification of documents and allocation of seat, bank draft of 18,000 US $ (eighteen thousand US Dollars) in favour of Dow University of Health Sciences is to be submitted alongwith the original documents as fee for first two semesters. alongwith admission fee 600 US dollars. 6 Time and Venue of the entrance test will be mentioned on the Admit Card. Incomplete Applications or those submitted after the last date will NOT be accepted. The Dow University has not authorized any institution or individual to conduct entrance test preparation. No personal contacts will be entertained.
