6 INSTRUCTIONS FOR FILLING THIS FORM Separate application form should be used for each discipline. Either TYPE or PRINT the entries on the form neatly. Go through the codes for Items given below and fill them carefully in the boxes on Page 1. All items marked with an asterisk (*) require you to submit an attested copy of some document. PLEASE DO NOT SEND ORIGINALS. Documents for which attested copies are required are given in the checklist. Please encircle the items in the checklist you are attaching with this form. Giving false information will lead to cancellation of admission. No column should be left blank. Write Not applicable against a column if the same does not apply to you.
