3 Pre-Participation Physical Examination Date: Monday Evening, April 17, 2017 Cost: $30-Cash Only Location: Ben Davis High School MPA and Outside Training Room Please be sure to pick up the special Physical form from the weight room, athletic office or athletic trainers. All physical examination forms that are to be used for the athletic seasons must be dated after April 1, Physical examinations administered by the doctors and staff of IU Health Athletes should arrive at the time that is scheduled for their class. Arriving before the schedule time will increase the amount of time that an athlete will have to wait in line. Each athlete who participates in this physical examination must have the personal information, medical history, student signature and parent consent portions of the examination form filled out before he/she enters the examination circuit. Athletes who will compete on a Ben Davis High School team are required to use one of the Ben Davis Pre-Participation Examination Forms and Ben Davis High School Student/ Parent Athletic Handbook that can be picked up at the Ben Davis Athletic Office and Main office beginning on May 16, 2017 This includes freshman competing on a high school team. Athletes must have a physical examination on record to participate in the summer conditioning programs. This rule was established by the principals of the Metropolitan Interscholastic Conference. 9 Ben Davis Youth Track Camp The Ben Davis Track Program continues to be one of the best in the state, developing athlete s abilities to sprint faster, run further, jump higher and longer and throw farther. All of these skills are elements to success in ALL sports, and BEN DAVIS TRACK promotes the multi-sport participation of the student-athletes at Ben Davis. The earlier an athlete learns how to run correctly, or to jump using proper rhythm and power, or to throw using total body forces, the sooner he or she will mature into a top level athlete. Ben Davis Track is proud of the tradition of success that track and field has brought to Wayne Township. Be one of our future state champions by signing up for THE TRACK CAMP and getting a head start on the Fundamentals of speed, power and endurance.
