2 July 1, 2015 Dear Student-Athletes, Parents, and Coaches, Our athletic program is designed to provide the student-athletes with unique learning experiences. We pride ourselves on having a competitive and successful athletic program, which involves over twenty-five percent of our high school student body. The emphasis of our program is based on dedication, desire and effort, rather than winning at all costs. It is imperative that the coaches, players, and fans of Lake Worth High School display sportsmanship, which will be remembered and valued long after the contests are over. This principle allows our athletic program to be kept in proper perspective in relation to the mission statement and philosophy of the school. The strength of our athletic teams comes from believing that giving one s best effort is the foundation for success, not only in athletics, but in all aspects of life. We believe a successful sports program enhances a solid education. Please read through our student-athlete handbook carefully, and contact the athletic office if there are any questions. 4 Student Athlete Guidelines Standards of Behavior: The Lake Worth High School Athletic Department prides itself on its outstanding reputation of sportsmanship. As a Lake Worth High School student athlete, keep in mind you represent not only yourself, your family, your team and also the entire Lake Worth High School Community. Therefore, any disregard for sportsmanship which includes but is not limited to outbursts of profanity or obscene gestures may result in your removal from practice or game. Any further offense may result in suspension or dismissal from the team. All student athletes must realize they are responsible for their actions while representing Lake Worth High School. Thus, any direct violation of the Honor Code, while representing the school as a studentathlete, may result in disciplinary action by the Athletic Director and/or the Administration. During an athletic contest all student athletes will be held accountable for any negative sportsmanship which exhibits a lack of respect toward opponents, officials, coaches or spectators. Penalties for such actions may merit a warning, a game suspension or the expulsion from the team, determined by the Administration, Athletic Director, or in conjunction with the head coach. The Florida High School Athletic Association may also impose further sanctions on any student athlete displaying negative sportsmanship. Medical Form and Consent and Release Form: No student athlete will be permitted to compete in any athletic practice or game until the Medical form and Consent and release form (required by the FHSAA) have been completed and returned to the athletic trainer. All pages must be signed by the doctor, parent and student in the appropriate places. Medical Excuses and Injuries: If the Athletic Trainer recommends a physician treats an injury, the student athlete must comply with the recommendation prior to returning to physical activity. If a student athlete is seen by a doctor, they will not be allowed to participate in either practices or contests until the player produces a note from the doctor releasing him/her from the doctor s care. However, all student athletes who are unable to physically participate with their team and are not being seen by the doctor or athletic trainer must still attend all practices and games or it will be considered an unexcused absence. Team Commitment: All Lake Worth High School student athletes may participate in only one sport per season (Fall, Winter, Spring), but are encouraged to participate in more than one sport throughout the year to develop into a more diversified athlete. Lake Worth High School student athletes are expected to attend all practices and contests of their chosen sport. If there is a commitment to teams other than interscholastic, as set forth by the Florida High School Athletic Association, the interscholastic practice and/or competition will ALWAYS take precedence. Once a student athlete has committed to participate in a given sport and has competed in at least one contest, he/she will NOT be permitted to join a different team until their original team has completed all competition. 5 Cut Off for Joining a Team: Weeks prior to the start of a particular sport, the head coach will have an informative meeting to discuss start times and rules. All student athletes who wish to try out for the team should make every effort to attend. No player will be allowed to join a team after the first regular season game or contest without special permission from the Athletic Director. However, earlier cut off dates are permitted at the discretion of the head coach. The exception to this rule will be student athletes who are finishing previous team commitments. Daily Attendance: Please consult the Lake Worth High School Student handbook for details on all attendance guidelines and procedures. To be eligible to participate in an athletic event, a student must arrive at school and attend his/her classes by 7:45am and may not sign out early from school. All student athletes must attend practice the day before a game to be eligible to participate in that particular contest. A student athlete who has an early dismissal due to illness will not be permitted to participate in any practice or game scheduled for that day. If a student athlete has three unexcused absences from practices or games a phone call will be made to the parents and a conversation will take place between the Athletic Director, Head Coach and studentathlete. In addition, the athlete will be suspended for the next game. Five unexcused absences and the athlete will be dismissed from the team. Early Dismissal for a Contest: Student athletes who miss class time due to an athletic contest MUST make arrangements with their teachers to make up any work they will miss and inform their teachers in advance in order to hand in assignments or make up scheduled quizzes and/or tests. Practice/Game Day Attire: Each team is given some form of practice attire which must be worn during practice time. Game uniforms are not to be worn at this time. Any student athlete wearing game uniforms during practice will be asked to leave practice until the correct practice uniform is worn. On game days, student athletes may wear their team jerseys to school, but they must be tucked into their school shorts or pants. The student athlete will lose this privilege if they abuse this rule. When uniform jerseys are inappropriate in style, white t shirts must be worn underneath them. Swimming, cross country, and water polo teams may wear their team t shirt. All team members should wear the same color jerseys. If this becomes an issue, then the privilege of wearing team jerseys on game days will be taken away. Code of Ethics for Student Athletes: 1. 6 3. Show respect to all players, officials, fans and coaches. 4. Refrain form the use of foul and abusive language. 5. Respect the integrity and judgment of game officials. 6. Refrain from the use of illegal and nonprescription drugs, steroids, or any substance that increases physical development or performance that is not approved by the United States Food and Drug Administration, Surgeon General or the American Medical Association. 7. Win with character and lose with dignity. Athletic Eligibility: Lake Worth Community High School is a member of the Florida High School Athletic Association and is bound by but not limited to its eligibility guidelines. If a student s Grade point average falls below a 2.0 for any semester, he/she will be ineligible to compete athletically for a period of 18 weeks, or one full semester. Lake Worth Community High School has a higher standard for athletic eligibility. Thus, a studentathlete may be placed on probation or may become academically ineligible to compete should he/she fail to meet his/her responsibilities as a student. High School Eligibility, Privileges, and Participation in Extracurricular Activities The High School strongly believes that a student s principle job is to be a scholar. Therefore, in the absence of a strong academic record, activities in athletics, extracurricular activities, and/or junior/senior privileges may be taken away or restricted. High School Academic Probation: Midway through each quarter, a snapshot of the grades will be taken and, if a student is struggling, he/she will meet with the appropriate administrator to determine a possible course of action. This could be the High School Administration and/or Athletic Director. Academic probation is a warning to a student that his/her academic performance does not meet the school s minimum requirements for participation. The probationary period allows the student an opportunity to improve his/her grades in a specific time period before becoming academically ineligible. Students placed on academic probation may continue to participate in all extracurricular activities. The High School Administration and/or Athletic Director may coordinate the development of an Academic Improvement Plan to help improve the student s academic performance. High School Academic Ineligibility: At the end of each quarter, a student may not have any of the following: A grade of F Two or more grades of D A grade of INC for any class In the unfortunate case that a student has one or more of the aforementioned grades, he/she will be deemed ineligible and will be unable to compete or participate in all extracurricular activities or receive junior/senior privileges. The student and his/her family will receive a letter signed by members of the Page 6 9 Updated: July 1, 2015

7 High School Administration advising them of the status of ineligibility. A student s academic performance will be reevaluated midway through the next quarter to determine whether or not he/she will remain ineligible. Students who are academically ineligible may not continue to participate in any extracurricular activities. High School Community Service and Privileges: If a senior or second semester junior is not current with his/her community service requirements, he/she will be ineligible to participate in any athletic extracurricular activities until this requirement has been fulfilled. The goal of Lake Worth High School s eligibility policy is to foster solid academic performance on the part of our student/athletes. Uniforms and Equipment: Student athletes will be responsible for all uniforms and equipment issued to them for use throughout the season. Any student athlete who has not returned any part of his/her uniform within five school days from the completion of their final contest will not be allowed to practice or participate in any contest until the uniform has been returned or paid for. Also, they will be unable to use the weight room, field or courts for personal use. There will be no exceptions! Disciplinary action may be taken and you may be placed on the obligation list at the discretion of the Athletic Director and/or Administration. 9 LAKE WORTH COMMUNITY HIGH SCHOOL STUDENT ATHLETE HANDBOOK VERIFICATION FORM This form (page) must be printed, signed, and turned into administration acknowledging that you have viewed and read the Lake Worth Community High School Coaches Handbook prior to beginning your coaching position. Note: updates to the Lake Worth Community High School Handbook may occur throughout the school year; for notification updates will be ed out to coaches and staff. 1. The entire eligibility packet MUST be completed and turned in to your coach or athletic department, prior to participation, which includes conditioning or try outs. There will be NO exceptions. 2. The eligibility packet is good for the entire school year and all sports played. 3. An athletic physical is good for the entire year. 4. The residency form and medical consent form MUST be notarized. 5. All student athletes will be required to pay a non refundable $10.00 fee toward insurance to try out or condition. 6. If the student makes the team, they are required to pay the additional insurance balance of $ This is mandated by the Palm Beach County School District.
