Your child will soon be enrolled in a school that will go above and beyond to ensure the highest quality of education is delivered in all areas of our curriculum on a daily basis. Our students and staff work diligently to create an environment that allows everyone to feel a strong sense of belonging and purpose. By signing this document, you acknowledge that your child must maintain acceptable levels of academic performance, attendance, and conduct in order for your child to remain enrolled in the Magnet Studio Program. You further understand that it is your responsibility to return this completed document to Miami Arts Studio Contract must be submitted at the assigned Magnet Parent Meeting: Middle School Students: March 20, 2018 at 6:30 pm and High School Students: March 21, 2018 at 6:30 pm. Failure to return this contract by April 6, 2018, as well as altering it in any manner other than as specified, will automatically mean that you decline the acceptance offer. 3 Magnet Contract Dear Parents and Students: Please review the following Miami Arts Studio 6-12 Magnet Contract. M-DCPS Grade Requirements for Magnet Students: Talented magnet class grade B or Above Academic class grade C or Above Conduct grade in all classes B or Above Effort grade in all classes 2 or Above An overall academic average C or Above Most of our students do not find it difficult to fulfill the above requirements. These requirements often serve as an incentive to the students to maintain higher standards. Very few students are dismissed at the end of the year due to poor grades. The magnet teacher will review report cards each grading period. A letter will be sent home with the student if his/her grades do not fulfill the grade requirements. These letters will communicate to the parent which steps need to be taken to ensure that the student s grade improves. Listed below are the three (3) notices in sequential order: After the first violation of a grading period: Probation Warning: This notice informs the student and parents which areas need improving. After the second violation of a grading period: Probation Contract/Conference: Students not meeting expectations will meet with parents, counselor, and lead teacher to assist student. The student may be denied participation on field trips, performances or exhibitions ( No Activity policy). After the third violation of a grading period: Final Notice/Exit Conference: A final parent conference is required at this point. The student will not be eligible to continue in the magnet program the following school year. The No Activity policy will be in effect. The student may or may not be allowed to finish the school year at Miami Arts Studio If the student is removed from the magnet program, he/she must return to their home school. Notification procedures may begin after the first violation of a grading period and will continue as necessary. 2

4 All students at Miami Arts Studio 6-12 must adhere to the requirements of Miami Arts Studio Zelda Glazer Student Handbook along with the following requirements: Demonstrate appropriate behavior, at all times, as outlined in the Miami-Dade County Public Schools Code of Student Conduct. Maintain school attendance of no more than 5 tardies/absences and early releases per semester (total 10). A student scheduled to attend a field trip or to participate in a performance or school sponsored event is required to attend school on the day of the trip, performance, or event. Performers who do not attend school on the day of the performance will receive an unsatisfactory absence. Students are also required to attend school the day before and after they participate in fieldtrips, performances and school-sponsored events. Students wishing to miss school for auditions, performances, or for other reasons need to bring a note from the parents to obtain a PRE-ARRANGED ABSENCE REQUEST from the principal and submit for approval the completed form a week in advance of the intended absence. These pre-arranged absences are a part of the 10 permissible absences allowed for the year. Approval will depend on the student s attendance, grades, and the total number of absent days requested in the year. Absences due to performances must be pre-approved by the principal. No more than a total of five (5) days pre-arranged absences will be approved. Performing arts students are required to fully participate in rehearsals, performances, fieldtrips, and competitions as designated by their arts faculty and must consider this participation a priority over any outside school activities. Failure to adhere to these guidelines may result in the student s dismissal from the program. I understand that my child is to adhere to the Miami-Dade County Public Schools Code of Student Conduct and Miami Arts Studio 6-12 Student Handbook. I understand that my child is to adhere to Miami Arts Studio 6-12 uniform policy. I understand that I am responsible for an annual magnet fee to cover costs associated with my child s magnet studio. I understand that Miami Arts Studio 6-12 does not provide transportation and that I am responsible for my child s transportation and timely arrival to school. The exceptions to the procedures described will be in the case of Level II or higher violations which could cause dismissal by the school principal. The dismissal will be immediate and the student will be transferred to his/her home school. 5 Miami Arts Studio 6-12 Dress Code Policy Polo: Black with MAS logo (must fit neatly, cannot extend below the elbow when arm held at side, or undersized; must extend below the waist line). Shirts (white or black) worn underneath the polo, must not extend beyond polo sleeve or below bottom of polo. Bottom: Khaki (cotton twill only). Pants must fit neatly and loosely. Pants must also be worn at the waist with a belt. No skinny-style pants are permitted. Athletic/nylon/denim/corduroy type pants are not permitted. High School MAS students may wear Khaki shorts in addition to the required uniform pants. Outerwear: Black jacket with MAS logo. Shoes: Only closed-toe-shoes are allowed. Spirit/Team/Club T-shirts: May be worn on Fridays as determined by the school administration. Announcements will be made. Please note that non-permissible clothing items/accessories include but are not limited to the following: No brand name/designer polo showing logos. No leggings, athletic wear, blue jeans/pajama pants, etc. No hats, bandanas, head scarves, or hoodies. No written messages, pictures, or symbols. No backless shoes, cleats, Crocs, house slippers, or shoes with wheels are allowed in school grounds. No trench coats. Students wearing any articles of clothing, jewelry, or hairstyles that may be construed as hazardous, distasteful or a distraction to the learning environment will not be permitted in school. Students are expected to come to school with proper attention given to personal cleanliness and neatness. Students whose personal attire or hygiene distracts the attention of others in school, will be required to make the necessary changes before entering the classroom or will be sent home by the principal and subject to appropriate disciplinary measures. 6 Media Release Without remuneration and as a donation of my service, time and/or talent to Miami Arts Studio 6-12, I hereby grant Miami Arts Studio 6-12, any related affiliate facilities involved with or licensed to the School Board of Miami-Dade County, Florida, and those representatives acting under its authority, permission to photograph, tape, record, broadcast, or display at will in print, audio, video, or a computer format my performance or participation. Full permission is granted to the editor/producer/representatives of the Miami-Dade County Schools to edit, make revisions or additions as considered necessary for printing, timing, or other program requirements, any such permission releases Miami Arts Studio 6-12 and its representatives from any liability for what might be deemed misrepresentation by virtue of editing, optical illusion or faculty mechanical or electronic reproduction.
