1 Application Packet Application Deadline: January 17, 2014 The me is a rigorous program that, if completed successfully leads to either an IB Diploma or IB Course Certificate. The requirements of the IB Diploma Programme are: Successful completion of all IB coursework (including Internal and External Assessments) in English, Second Language, Individuals and Societies, Math, Experimental Sciences and an IB Elective (The Arts or another course in subject groups 1-5) Completion of the Theory of Knowledge course Completion of CAS (hours/projects in Creativity, Action/Active and Service) Completion of an independent 4000-word Extended Essay The requirements of the IB Course Certificate are: Successful completion of all IB coursework (including Internal and External Assessments) in the chosen course(s) Checklist for application process: Completion of the application form IB Course selections, include completed (or in progress) prerequisites, grades and teachers o IB Coordinator will obtain teacher recommendations Academic Agreement and IB Code of Conduct review and parent/guardian and student signatures Response to the writing prompt The writing prompt response will be completed at Quarry Lane during advisory period on Wednesday, January 22. All students are required to attend and complete their writing prompt during this time. If English is your second language, completion of English 1 (or in progress) and most recent TOEFL score 1

3 Course Selection and Prerequisites Please select the IB courses you would like to take in 11 th -12 th grade, using the following guidelines: You must select ONE two-year course in groups 1-5, the sixth course can be in group 6 or another group. You must take at least THREE courses at a HIGHER LEVEL (HL) and THREE courses at a STANDARD LEVEL (SL). Very strong candidates can choose up to 4 HL courses (with 2 courses at a SL). In Group 3, only History HL fulfills the UC system US History requirement. None of the Group 3 courses fulfill the UC system World History Requirement. UC s require both US History and World History. If your TOEFL score is below 95, you may take an English course during the second semester or over the summer and will need to re-take the TOEFL before school starts. As soon as we receive your TOEFL score, a decision will be made on your IB application. 4

5 Academic Agreement Continued participation in the IB program is dependent on academic performance. Students must maintain an unweighted GPA of 2.5 in all IB courses and may not receive more than two "C-'s" or any "D's" or "F's" on their semester grades. If this occurs, the student will be placed on probation. IB Students are allowed one probationary period while in the IB program. The next infraction could result in a dismissal from the IB program. I have read and understood both the requirements of the me, the Quarry Lane requirements and the consequences of failing to continue to meet the minimum grade point average once admitted to the IB program at Quarry Lane School. 6 Academic Malpractice and cheating: IB defines malpractice in the following way: IB Code of Conduct Malpractice: behavior that results in, or may result in, the candidate or any other candidate gaining an unfair advantage in any assessment component. It may include: 1. Collusion: this is defined as supporting malpractice by another candidate, as in allowing one s work to be copied or submitted for assessment by another. 2. Plagiarism: this is defined as the representation of the ideas or work of another person as the candidate s own. 3. Duplication of work: this is defined as the presentation of the same work for different assessment components and/or IB diploma requirements. 4. Any other behavior that gains an unfair advantage for a candidate or that affects the results of another candidate (for example, taking unauthorized material into an examination room, misconduct during an examination, falsifying a CAS record, disclosure of information to and receipt of information from candidates about the content of an examination paper within 24 hours after a written examination). Quarry Lane subscribes to Turnitin.com, a website that checks for plagiarism. When students submit work through ManageBac (an online IB platform for certain classes, CAS, TOK and Extended Essay), their work may be submitted to Turnitin.com. Teachers may also submit work to Turnitin.com. The IB organization also uses Turnitin.com and we do not want any work returned to due to malpractice. Collaboration and group work can be effective and is allowed for certain assignments with teacher permission. Quarry Lane s IB Program places an emphasis on student, parent, and teacher commitment, responsibility, and motivation. This rigorous program requires a concerted and constant effort on everyone's part. Parents and students are expected to attend regular mandatory meetings with teachers and the IB coordinator to discuss coursework, assessments, and due dates. IB dates and due dates cannot be altered. Late work is not accepted and the result is the loss of a chance to earn an IB Diploma or Course Certificate. I understand Quarry Lane s policy on cheating, behavior and attendance as they pertain to the IB program. I pledge to uphold high standards throughout my academic career.
