1 TIMBERVIEW MIDDLE SCHOOL ATHLETICS MICHAEL ARSENAULT, ATHLETIC DIRECTOR CRYSTAL ROSS, ATHLETIC SECRETARY 7-8 Girls SOFTBALL Timberview Middle School s softball program is committed to skill development, participation, team work, and good sportsmanship. The participation fee for softball is $40. If this presents a hardship for you, contact the athletics office at A valid current sports physical (within the past 12 months for the entirety of the sport season) MUST be on file before a player is allowed to practice or participate. Physical forms are available from the athletics office or on our website. A standardized form from your physician is acceptable. The signed registration consent form, fee, and physical should be turned in to the athletic secretary BY NOON the first day the student attends practice. An athlete who is injured, seen by a physician, and cannot practice for an extended period of time must provide a doctor s statement saying he/she is cleared to play all sports before he/she will be allowed to participate in any sport. Students will ride a bus to away games during the week, but parents are responsible for picking students up from those meets. There is no bus back to Timberview. Parents are responsible for all transportation on weekends. Athletes may not ride home with friends without prior written permission from their parents. Students are expected to attend all practices and games and to be on time. If a player will miss a practice or game, a parent should leave a message for the coach at The grades of interscholastic athletes will be checked on Tuesdays for academic eligibility each week. Students with two or more D s or one or more F s will be considered ineligible to compete in games the following week unless they complete the Eligibility Form. The form requires an improvement plan and signatures from the teacher(s) and a parent and must be returned to the athletics office by Friday at 3 p.m. Ineligible students are permitted to practice but may not play in games or travel with the team to away games. A replacement fee will be charged for all lost or damaged equipment/uniforms. Athletes will not be eligible to play another sport until all equipment and uniforms are returned and all applicable fees are paid. Athletic Fee Refund Policy Refunds are only available during the first two weeks of practice. Athletes withdrawing during the first two weeks of practice will be given a refund ONLY after they have submitted a drop slip signed by a parent/guardian and coach. Drop slips are available in the athletics office. Athletes withdrawing after this time will not be issued a refund. Timberview will provide: Game uniform Softballs Bats Athletes must provide: Athletic shoes or softball cleats Softball mitt Lock for gym locker Water bottle Appropriate workout attire

Please be sure your student s information is up-to-date. SPORTS CONSTITUTION AND PERMISSION TO PARTICIPATE I, the parent or legal guardian of the above named student, hereby give my consent for my child to participate in Timberview Middle School s sports program for either the duration of the sport or until my student chooses to quit the sport, and I hereby release the school district and hold it and its employees harmless against any liability for injuries my student may incur as a result of participating in the sport. I have read and understand the rules and regulations of the sports constitution (located on the Timberview athletics website). I understand that my child will be held to these standards and consequences, and I have clarified any questions that I may have with the coaches. I understand that this document can be changed at any time at the discretion of the coaches or administration, and all decisions made are final. I have read and agree to the SOFTBALL registration information attached to this document.
