1 Student College Planning Form To receive the most benefit from our meeting, you should gather and organize some information ahead of time. Complete this form and bring it, along with your high school transcript and/or a resume, to our meeting. Besides your academic performance and personal accomplishments, there are other attributes that interest college admission officers. Before we meet, give some thought to the following questions: How will you describe your ethnicity? Have you been burdened with any major obstacles during your high school years? Did either of your parents attend college? Are you planning to apply Early Decision to a college? Remember, Early Decision is binding. This means that, if you apply Early Decision and get admitted, you are required to attend. You may only apply Early Decision to one college. Are you being recruited, or are you reasonably certain that you will be recruited, by a college coach for your athletic ability? Academic Record Much of this information can be found on a recent transcript. 1. Enter your unweighted cumulative GPA below: Your unweighted cumulative GPA is your high school GPA to date before applying a multiplier, or adding points, for honors, AP (Advanced Placement), or IB (International Baccalaureate) courses. If only your weighted cumulative GPA appears on your transcript, estimate your unweighted cumulative GPA by subtracting points awarded for honors, AP, and/or IB courses. Unweighted Cumulative GPA 2. Enter your class rank, if available, below: If your high school does not rank students by number (e.g., it ranks students by decile), make your best estimate of where you stand academically in relation to your classmates. Enter your current core courses, or the core courses you are scheduled to take in the upcoming academic year, by name and level below: The five core subject areas are: English, mathematics, science, history or social studies, and foreign language. The levels are regular, honors, AP (Advanced Placement), IB (International Baccalaureate), or college-level. If you are not currently taking, and are not scheduled to take one or more of the core courses, simply leave blanks as appropriate. 4 Personal Record Colleges will consider what you have done outside the classroom too. 1. List any individual talent below: Be sure to indicate the level of your accomplishment.

List your community service or other volunteer efforts below: If you have served your community or otherwise volunteered to help others, estimate, and record below, where you helped out and the number of hours you have contributed in the last 12 months.

6 Colleges It is important that you have at least a preliminary college list before we meet. If you are far enough along in your thinking, in the Apply List list the colleges to which you plan to apply. In the Consider List, enter a list of colleges you are considering, but to which you may or may not apply. Finally, your parents can list the colleges they think you should be considering in the Parent List.
